Seeking Independent Contractors to Become Family Home Providers

WHAT WE DO?

Life Works Residential provides individuals with intellectual and developmental disabilities a stable, supportive, inclusive home environment with one of our Family Home Providers. We recognize family is not one size fits all and we work to ensure individuals have the opportunity to explore several home options, roommate options and ultimately choose the family dynamic that best fits their needs and desires. Individuals residing with a Family Home Provider experience family household routines, holidays, vacations and a variety of fulfilling life experiences. This naturalized living environment provides an opportunity to acquire new skills, improve on or retain present skills and explore new experiences. For many individuals this is an opportunity to develop an extended family and for others to develop family relationships they have never had. Residing with an FHP offers a level of security and stability that is key to the achievement of personal goals and dreams.

WHERE ARE FAMILY HOMES LOCATED?

Life Works Residential offers Family Homes throughout Kentucky and are always open to considering new areas of the state if there is a need.

WHAT DOES A FAMILY HOME PROVIDER DO?

The responsibilities of a Family Home Provider vary depending on the level of support required by the individual. Examples of support include assistance with activities of daily living, assisting with scheduling and attending medical appointments, medication administration, household tasks, socialization and community involvement, etc. Family Home Providers contracted with Life Works Residential have the option to support up to three individuals with intellectual and developmental disabilities in their personal home, depending on the number of bedrooms/bathrooms and the amount of shared space that is available.

WHAT ARE THE REQUIREMENTS TO BECOME A FAMILY HOME PROVIDER?

  • Be at least 18 years old with a high school diploma/GED OR be at least 21 years old.
  • Have the ability to communicate effectively with the individual and their support team.
  • Have the ability to read, understand, and implement written and oral instructions.
  • Perform required documentation
  • Participate as a member of the participant’s person-centered team if requested by the participant
  • Demonstrate competence and knowledge on topics required to safely support the participant as described in the participant’s person-centered service plan
  • Possess a home with adequate bedrooms, bathrooms and shared space for current family members and potential individuals.
  • Possess a personal, reliable vehicle and a valid Kentucky driver’s license.
  • Ability to physically carry out duties of position.

  • Excellent compensation, inclusive of monthly room & board contribution per individual supported.

Job Type: Contract

Pay: $39,800.00 - $119,000.00 per year
