Title: Nurse Practitioner (Primary Care and MAT)

Department: Admin Supervisor: Medical Director

Objective: We are seeking a highly skilled and compassionate Nurse Practitioner to provide primary care services to both children and adults, including sick visits and well visits. Additionally, the ideal candidate will be able to provide Medication-Assisted Treatment (MAT) for individuals with Opioid Use Disorder, including prescribing Suboxone to qualified patients. This role will require strong communication skills, a dedication to patient care, and the ability to work collaboratively with a multidisciplinary team.

Responsibilities/Duties:

  • Conduct comprehensive physical exams for both children and adults, including taking medical histories and ordering diagnostic tests as needed.
  • Diagnose and treat acute and chronic illnesses, injuries, and infections, while promoting preventative care and wellness.
  • Provide medication management and prescribe medications as appropriate for the patient's condition, including Suboxone for individuals with Opioid Use Disorder.
  • Monitor patient progress and adjust treatment plans as necessary, while maintaining accurate and timely medical records and documenting all patient interactions.
  • Educate patients and their families on preventative health measures, treatment options, and disease management.
  • Collaborate with other healthcare professionals, including physicians, nurses, social workers, and case managers, to ensure comprehensive and coordinated care.
  • Ensure that all documentation accurately reflects the services provided, the level of complexity of the encounter, and the time spent with the patient.
  • Seek consultation regarding high-risk cases with collaborating physician to ensure patient is receiving appropriate level of care.
  • Respond to crisis situations experienced by patients served (contact patients within 24 hours of crisis).
  • Attend meetings with collaborating physician(s) and continuing education trainings per licensing board requirements.
  • Adequately address frequency of client no-shows.
  • Attend monthly provider meetings for case consultation and administrative updates.

Required Skills/Education/Experience/Certifications:

  • A current, unrestricted North Carolina Nurse Practitioner license and DEA registration.
  • Board Certification as a Nurse Practitioner in the relevant specialty.
  • Minimum of 2 years of experience in primary care and medication management, with experience providing MAT preferred.
  • Strong clinical skills and knowledge of evidence-based practices in primary care, medication management, and MAT.
  • Excellent communication skills and the ability to establish rapport with patients and their families.
  • Ability to work collaboratively with a multidisciplinary team and adapt to changing priorities and situations.
  • Commitment to providing high-quality, patient-centered care.
