Foster Care Home Support Specialist Job at Saint Francis Ministries
Ø Foster Care Home Support Specialist will complete assessments of foster parents for placement of children.
Ø Foster Care Home Support Specialist will completes written documentation and licensing requirements per DHHS guidelines.
Ø Foster Care Home Support Specialist will determine and recommends services needed by the child and family to maintain placement.
Ø Foster Care Home Support Specialist will provides training regarding foster care, as needed.
Ø Completes support plans, as needed, with foster care families.
Ø Completes and maintains written documentation in child/family’s case record accurately and within established timeframes.
Ø Participates family team meetings when invited, sharing pertinent information about children/families’ progress, collateral contacts, and relationships with other agencies/communities.
Ø Provides intervention for clients in crisis.
Ø Serves as a Liaison between foster parents and the Nebraska DHHS Case Managers
Ø May participate in court proceedings as requested
Ø Supports families to maintain placements and help children and families work toward permanency.
Ø Participates in office team meetings.
Ø Reports critical incidents to DHHS and to St. Francis Ministries in accordance with required timeframes and direction.
Ø Implements Saint Francis Ministries policies and follows directives as required. Follows and adheres to all pertinent Saint Francis Ministries Standard Operating Procedures (SOP’s), rules, personnel policies, and procedures; related accreditation and licensure standards; and federal, state, and local rules, statutes, regulations and contractual terms.
Ø Ensures clients’ rights are protected. Makes sure all clients have received their rights and maintains a signed copy in each file.
Ø Is knowledgeable of and follows all safety procedures.
Ø Reports unusual incidents through appropriate Risk Management, clinical and safety channels.
Bachelor's required.
