Reliant at Home is seeking a Full-Time HHA to cover Houston, primarily the Katy/Cypress/Jersey Village areas. This opportunity has an expectation of ADL pt care Mon-Fri with flexible hrs.
Hospice experience preferred. Consolo EMR experience a Plus!

Responsibilities
Primary functions are to perform pt ADLs- Report to Case Manager change of pt status.
- Document pt visits daily
- Timely submission of all required paperwork.
- Demonstrate commitment, professional growth and competency.
- Meet productivity standards.
- Provide effective communication to patients and their families, team members, and other health care
Requirements:
- Current CNA certification
- Two years experience as a CNA in a clinical care setting and 1 year full time hospice experience.
- Reliable transportation. Valid and current auto liability insurance.
- CNA skills as defined as generally accepted standards of practice. Good interpersonal skills preferred in palliation of end-of-life. Proof of current CPR. Must read, write and comprehend English.
