Position Summary
The position works collaboratively across systems to provide overall management and coordination of a patient’s recovery care plan for individuals with substance abuse disorders, medical needs/chronic health issues, and co-occurring disorders in an integrated care setting. The position is grant-funded through 2025. All duties and responsibilities will be performed in accordance with the DCCSB Mission and Vision statements.
Essential Duties:
Review SBIRT, conduct ASAM assessments, and develop treatment plans. Provide individual and group counseling as needed. Make referrals as appropriate.
Identifies any barriers clients experience in accessing health care and facilitates the development of recovery care plans with those who have comprehensive recovery support needs across disciplines and programs. Includes consulting and advocacy with health care teams, and partner agencies.
Recognizes and employs the role of cultural, social and behavioral factors in the accessibility, availability, acceptability, and delivery of public health services.
Uses individual, team and organizational learning opportunities for personal and professional development.
Reports suspected abuse or neglect to appropriate authorities.
Maintains documentation, collects data and submits reports as required and in a timely manner; includes screenings, assessments, progress notes and case management.
Serves as a role-model and educates clients about available services and resources. Performs all duties with nonjudgment, confidentiality, cultural awareness, and sensitivity.
Recognizes and responds appropriately to crisis and emergency situations. Evaluates individual’s satisfaction with progress toward recovery and wellness goals.
Maintains current, accurate knowledge of trends and issues related to wellness and recovery. Recognizes and maintains personal and professional boundaries.
Other Duties:
Chemical Dependency Delegation of Authority as needed. Other duties as assigned.
Knowledge, Skills and Abilities:
Ability to understand and follow Behavioral Health standards and grant guidelines.Demonstrates continuous development of trauma-informed clinical skill sets.
Understanding of and sensitivity to alcohol, other drug use, and the recovery process. Understanding and support of trauma-informed care.
Recognizes the role of cultural, social and behavioral factors in the accessibility, availability, acceptability and delivery of public health services.
Ability to employ individual, team and organizational learning opportunities for personal and professional development.
Understanding and support of MAT and Peer Delivered Services as critical components to SUD treatment. Understanding of grant administration.
Basic computer skills with ability to effectively use MS Office Suite programs Strong oral and written communication skills
Conducts self in a professional manner and maintains strict client confidentiality Familiarity with Native American/Alaska Native cultures and the CTUIR community
Typical Physical Demands:
Requires prolonged sitting, some bending, stooping and stretching. Requires eye-hand coordination and manual dexterity sufficient to operate a keyboard, photocopier, telephone, calculator and other office equipment. Requires normal range of hearing and eyesight to record, prepare and communicate appropriate reports. May require contact with individuals with communicable diseases.
Typical Working Conditions:
Work is performed in a medical/clinical environment, as well as some community-based work. Involves frequent contact with staff, patients, and the public. Work may be stressful at times. Contact may involve dealing with angry or upset people.
Other Aspects of the Position:
Requires occasional weekend and/or evening work with variable shifts
May require travel for training and meetings and/or driving and patient transport.
Minimum Qualifications:
CADC I required.
Basic computer skills.
Excellent communication skills, including the ability to communicate across disciplines.
Must possess a valid driver’s license and maintain insurance requirements to operate General Services Administration vehicles.
Must submit to and pass a reference and criminal background check. Must submit to and pass a pre-employment drug and alcohol screening.
Various Work Schedule: Monday-Friday 9a-5p or 12p-8p. Available to work weekend hours, as needed.
Job Type: Full-time
Pay: $35,000.00 - $40,000.00 per year
