Guardianship Coordinator Job at The Arc of New Mexico
Will be required to successfully test and obtain a Center for Guardianship Certification (CGC) within 90-days of employment with The Arc of NM (per new regulations). The Arc pays for the test and all required CEU's.
$35,568-38k, DOE plus $1,000 annual on-call stipend.; plus benefits
POSITION PURPOSE
The purpose of the Guardianship Coordinator position is to provides support and guidance for decision-making at the level each individual Arc client requires around living, working, learning, recreation, leisure, healthcare, medical, behavioral, therapy, medical and safety; per the National Guardianship Association (NGA) Standards. May also assist individuals in the development of relationships (family, friends, and partners) within the larger community.
Performance Measures: A successful Guardianship Coordinator will produce positive outcomes in the following areas: quality services of guardianship support to clients, achievement of ensure the rights of clients are maintained and expanded while maintain and protecting confidentiality.
ESSENTIAL DUTIES AND RESPONSIBILITIES (IN PRIORITY ORDER)
- This position requires you to be on call 24/7. Emergency calls may occur after “normal” office hours.
- Travel outside of the Metropolitan area to SE/SW/NE/NW quadrants will be required on a minimum of one time per month, depending on client needs. One overnight stay may be required.
- Safeguards the human and legal rights of persons served through a regular review of services, regular contact with persons served and representation with/or for persons served in legal and administrative setting as well as within programmatic activities.
- Participates in Individualized Service Plan and program meetings to assure proper program development and implementation and to assure that the individual’s needs are addressed.
- Participates in medical treatment and medication decision-making on behalf of all persons served and provides informed consent on behalf of all individuals.
- Insures that individual program eligibility is established and maintained and that each person served is referred to or receiving all services as appropriate.
- Oversees the property and finances of each person served through regular review of financial information and the establishment, as needed, of other entities such as a representative payee, conservator or trustee.
- Monitors all individual environments, services and supports of each individual served through regular (monthly) visits to homes, workplaces, recreational areas, habilitation services and through other contacts such as meetings, appointments, and evaluations.
- Maintains files on each person served that contains demographic data, guardianship information, program information, progress information, evaluation and medical and social history.
- Provides regular reporting to District Court as required by NM State Statute #45-6-301 for persons served.
- Develops and maintains a positive working knowledge and relationship with all service providers and agencies.
- Mentors individual guardianship coordinators and others, as needed, with the development and transition from other DD services to the Mi Via waiver.
- Participates in training, workgroups, stakeholders meetings and planning at The Arc to acquire knowledge and enhance expertise in the operation of Arc initiatives such as Mi Via, Designing Dreams and other activities.
- Provides support to guardianship and other programs of The Arc to promote individual choice, self-determination and person-centered planning and to explore the use of alternative approaches to provide supports to persons with developmental disabilities throughout New Mexico.
- Protects the confidentiality and rights of all persons served or employed by the Arc of New Mexico.
- Promotes a positive image of The Arc of New Mexico and those persons it represents.
- Monitors all individual environments, services and supports of each individual served through regular (monthly) visits to homes, workplaces, recreational areas, habilitation services and through other contacts such as meetings, appointments, and evaluations.
- Apply the use of technology to enhance the efficiency and quality of service.
Other Duties as Assigned and that meet the minimum qualifications of this position.
EDUCATION& RELATED WORK EXPERIENCE
Education Level/ Years of Related Work Experience:

Bachelor’s degree preferred with guardianship experience. Experience with intellectually and developmentally disabled preferred.
Skills and Knowledge

WORK ENVIRONMENT
Routine office environment. Regular work week with occasional early evening or weekend hours.
CONDITIONS OF EMPLOYMENT
Must be able to pass comprehensive background check. Must access to reliable transportation, valid ID/driver’s license, and meet the required automobile insurance minimums for the State of N.M.

Physical Requirements:
Ability to travel, sit for prolonged period of times. The position requires the ability to lift 20 pounds unassisted, 40 pounds with assistance. Travel may require driving in inclement weather or in the evening after sunset.
