As a Health Guide at DayTwo, you will work alongside a Registered Dietitian to empower members to take an active role in their health to live longer, happier lives. DayTwo has a unique care team approach that provides support to our members both from a Registered Dietitian and a Health Guide.
General/ Position Summary:
DayTwo is growing quickly and we are seeking experienced Health Guides to support our members in achieving better health. The Health Guide aligns with the member in order to set goals and achieve behavior modification using a coaching model. Skills utilized include Motivational Interviewing, Positive Psychology and goal setting with the member. The Health Guide functions as part of a care team as the behavior change agent and owns the member relationship.
This is a remote role that requires the ability to learn and use a variety of platforms in order to implement the program. The ideal candidate must be able to work independently while still collaborating with team members across all channels. Comfort with technology is a must in this role. This is an exciting position in a dynamic start-up environment.
- Due to the high demand for evening appointments, two evenings/nights (until 9pm EST) are required for this role.
Responsibilities:
- Use proven coaching techniques to guide members along their health journey.
- Engage members and develop rapport to support healthy behavior change.
- Support members in goal development and provide accountability.
- Meet required performance metrics and individual goals.
- Monitor and respond to members on chat channels in a timely manner.
- Maintain company competencies and policies.
- Maintain direct and clear communication with RD, members, team, providers, and manager.
- Actively participate in, and follow, recommendations for professional growth opportunities and coaching.
- Meet evolving company objectives by helping with other special projects or assignments as business needs dictate.
Requirements:
Qualifications and experience:
- Bachelor of Science preferably in a health-related field
- Previous health coaching experience
- High comfort level with technology including proficiency using Google tools
- Reliable internet connection
Preferred
- Health Coach and/or NBC-HWC certification
- Experience working with diabetes in a telehealth environment
- Bilingual
