JOB SUMMARY:
The Level II Health Information Technician performs clerical and chart management functions necessary to the operation of the Health Information Department. Level II Health Information Technician cross trains and provides coverage for Level I HIT as needed. Level II Health Information Technician abstracts paper and electronic data to complete the patients’ medical record, as well as facilitates secure release of patient information for continuing care and insurance requests.

ESSENTIAL FUNCTIONS, DUTIES, AND RESPONSIBILITIES:
- Release of information for continuing care and primary insurance requests in accordance with established hospital policy and federal and state legal requirements
- Analyze all hospital and clinic encounters in accordance with established criteria, policies and procedures.
- Assist with the quality/compliance chart audits in accordance with established criteria, policies and procedures.
- If designated, act as the physician deficiency clerk to notify and alert clinical staff per Livingston HealthCare policy.
- Maintain birth certificates, paternity papers on all newborns and report this information to both the County and State.
- Process death certificate paper work and maintain record on file.
- Oversees charts storage areas and prepares charts for destruction in accordance with the Destruction Schedule.
- Assists in developing written job procedures facilitates cross training for coverage needs.
- Crossed trained to perform all Clerk I job functions, duties and responsibilities Demonstrates attention to detail, accuracy, and data integrity.
- Perform other duties as assigned.
ADDITIONAL RESPONSIBILITIES:
- Attends orientation and other compliance/privacy/security/educational sessions.
- Maintains strict adherence to confidentiality and HIPAA requirements.
- Participates in continuous quality improvement.
- Performs other duties as required or requested.

QUALIFICATIONS (Required):
- High school diploma (or equivalent) is required.
- Knowledge of basic office functions.
- Good verbal and written skills with attention to detail.
- Must treat patient, employee, clinic and hospital information as confidential.

ADDITIONAL DESIRABLE QUALIFICATIONS:
- Previous experience in Health Information Management or healthcare office setting.
