Home Care Aide Coordinator

Living Care Home Services (LCHS) is currently seeking a Home Care Aide Coordinator to work full-time on-site in our Doylestown, PA, office. The person that fills this position will help to achieve our mission of providing families with compassionate and quality in-home care.

Responsibilities

Work as part of a team to coordinate services for clients by…

  • helping prospective client(s) during initial phone call for service.
  • meeting with family and/or client(s) to discuss and prepare care plan.
  • scheduling caregivers to meet family’s needs; includes all planned and unplanned requests for time off.
  • assisting with on-site introductions between caregivers and families. This may include supplying transportation for live-in and/or hourly caregivers.
  • maintaining relationships with both families and caregivers through phone and personal visits.
  • assist in keeping client/caregiver Registry and files on computer platform as well as paper versions up-to-date.
  • tracking client/caregiver schedules via spreadsheet program.
  • attend weekly team meetings and provide updates from weekly events.
  • be an active team member of on-call rotation schedule and represent LCHS during evening and weekend hours by providing assistance to clients and/or caregivers.
  • project a professional image and demonstrate compassion when dealing with others.

Qualifications

  • Compassion and empathy for others.
  • Excellent communication skills.
  • Case management experience which includes working with seniors in need of care. Knowledge of Alzheimer and dementia needs a plus.
  • Experience with creating and managing multiple schedules preferred.
  • 1-2 Years of experience working with DOH Home Care Registry or Agency Regulations would be preferred.
  • Time management skills and ability to multitask under pressure
  • Working knowledge of MS Office (Word, Excel and Outlook)
  • Active Driver’s License with a clean driving record
  • Reliable mode of transportation
  • College degree from an accredited institution preferred
