Our team is Hiring! We are searching for a Homecare Case Manager who is also comfortable working in Sales and can assist in Intake. Ideal candidate will have experience working in similar roles within Homecare, Home Health, or Hospice.

Qualifications:
- Homecare Case Management experience preferred
- Familiarity in intake is a plus
- Home Health/Hospice/Homecare experience preferred
- Sales motivated, patient care driven
- Highly Accurate Data Entry
- Ability to comfortably and effectively coordinate care
