Home Health and Hospice Sales Representative Job at Westminster Canterbury Richmond at Home LLC
The Home Health & Hospice Sales Outreach Coordinator is responsible for planning and implementing marketing and sales activities for Westminster Canterbury Richmond at Home (WCRaH) clinical programs. This role contributes to achieving monthly start of care (SOC) and average daily census (ADC) goals for Home Health and Hospice service lines through initiating contact with and responding to inquiries from prospective residents, hospitalized patients and referral sources; to develop and expand patient census via phone calls, scheduled appointments and walk in or unscheduled meetings with key stake holders in hospitals, physician offices, skilled nursing facilities and assisted living facilities. Coordinate all steps in the referral to admission process for new or returning patients to include meeting with families and residents, hospital case managers, referring physicians and office nurses.
