The intake Coordinator is responsible for accepting and processing of all patient intake referrals. They are responsible for verifying insurance, assigning patient start of care and disseminating admission information to all appropriate personnel.

Responsibilities:

  • Receives patient intake calls, faxes or electronic requests from referral sources. Determines services requested, payer source, general location of client.
  • Process patient referrals demographics and financial reimbursement information.
  • Verify eligibility and benefits of Medicare, Medicaid, Commercial and Managed Care Insurances.
  • Verification of patient MD and last appointment held.
  • Daily data entry of documents and maintenance of patient clinical information.
  • Tracks pending referrals for facility Discharge
  • Manage patient Face to Face information
  • Tracks re-hospitalized clients for status and expected home care resumption of care.
  • Calls client after facility discharge to welcome to CHHC and describe services ordered and scheduled.
  • Acts as a back up to Clinical Benefits Verification Coordinator.

Qualification Requirements:

  • Excellent customer service skills
  • Highly organized and able to manage high volume of calls and tasks
  • Knowledge of insurance contract requirements, as well as federal, state and local regulations.
  • Knowledge of agency policies and procedures.
  • Proficient in use of computer, telephone, voice mail, fax and other office equipment.
  • Able to travel by car to designated meetings as required.
  • Able to promote strong interpersonal relationships and effective communication skills.
