Position Essential Duties/Responsibilities
- Provides direct skilled nursing services to clients within the scope of practice of a licensed practical nurse.
- Assists with development of written plans of care with client and/or family and assures understanding and compliance with team members.
- Routinely evaluates client and effectiveness of care plan, and participates in client treatment planning and case review with client care providers.
- Routinely provides care and instruction to client and care providers both on campus and in the community.
- Responsible for medications and treatments, and reviews all required records for completeness and accuracy. Provides support to caregivers as needed.
- Assists with direct care, such as bathing, toileting, feeding, and other client assistance as needed.
- Obtains feedback from clients and families to ensure satisfaction with services and quality of care as needed.
- Teaches, supervises, and counsels the client, staff and family regarding care needs of the client within their environment.
- Assists in the collaboration with multidisciplinary team members to coordinate care and initiates referrals to community resources (i.e. Hospice, Medicare Home Health) as necessary.
- Assists with educations of staff members on skill competency and performance requirements as needed to assure safe practice.
- Attends family meetings as needed to facilitate client support.
- Assists with monitoring services for clients on campus and in the greater community.
Position Qualifications
- Current license as a Licensed Practical Nurse in the Commonwealth of Pennsylvania required.
- Minimum 3 years of nursing experience required.
- Has and maintains current CPR certification.
- Maintains current valid Pennsylvania driver's license.
- Geriatric or home care experience preferred.
