Position Summary: The Outreach Case Manager will work throughout San Mateo County identifying unsheltered homeless people residing in homeless encampments and other places not meant for human habitation. The Outreach Case Manager will utilize a multidisciplinary approach to engage the hardest to serve homeless in an effort to connect them to services. Responsibilities include providing outreach and intensive case management to unsheltered homeless individuals, perform screening assessments, make appropriate referrals, connect clients to social services, and keep up-to date case files.
Case Management Duties:
- The Outreach Case Manager will be in the field, in person 80% of the time
- Coordinate with medical care team to understand client’s immediate needs
- Keep care team up to date with client's medical statuses, additional needs and any follow up care needed
- Assist in scheduling medical appointments; including transportation; and medical appointment accompaniment
- Act as the point of contact for each client, if they need it, at medical facilities
- Transport equipment, medication and or test results on behalf of the medical care team
- Provide relevant resources, health information and linkages based on individual health care needs
- Keep up to date case notes and data
- Develops, implements, and revises individual case plans and consistently monitors progress toward client goals
- Provide crisis management for clients; makes linkages for interventions as appropriate Attend weekly case consultation meetings and monthly training; meet regularly with supervisor to exchange pertinent information and receive supervision
- Provide on-going supportive and/or case management functions in accordance with the case/treatment plan
- Maintains a high level of ethical conduct regarding confidentiality, dual-relationships, and professional stature Position Qualifications
- Bachelor's degree in social work (preferred), psychology or related field, with a minimum of one years' case management experience
- Excellent communication and interpersonal skills; proven ability to collaborate with community partners
- Ability to maintain a high level of ethical conduct regarding confidentiality, dual-relationships, and professional stature
- Understanding of the issues faced by homeless individuals and other marginalized populations in crisis
- Availability to work evenings as needed
- Experience working with mentally ill and dual diagnosed clients highly desirable
- Bi-lingual Spanish language capabilities a plus

- Prefer Undergraduate degree
- Professional experience in working with homeless individuals
- Demonstrated ability working with individuals from diverse ethnic and socioeconomic backgrounds
- Comfortable interacting and communicating with medical professionals
- Excellent written and oral communication skills; comfortable presenting in front of groups
- An understanding of, or willingness to learn about, ideas and practices that promote diversity, equity, and inclusion
- An understanding of ideas and practices that promote diversity, equity and inclusion, and a commitment to ongoing professional development in this area.
- Strong organizational, detail oriented, and time management skills.
- Proven ability to work independently and effectively as part of a collaborative team
- Displaya high level of initiative and commitment towards completing duties efficiently
- Valid California driver’s license and proof of insurance
