Hospice Admissions RN Job at Salmon Health and Retirement
Admissions - Hospice RN needed to complete admission to hospice for patients and their families/caregivers. You will be providing education to patients/families and enhancing an environment that is cohesive to end of life care. During each admission visit you will conduct clinical assessments, collect information from the patient and/or their family, and establish the patient's initial hospice plan of care. You'll provide compassionate, active listening, detailed documentation on initial assessment, and help patients and families understand the benefits and supportive nature of hospice care. You will be working with a supportive team, and have the ability to make a positive impact for patients and families.
