If Hospice is your passion, WE NEED YOU ! Looking for a dedicated Hospice Nurse with a strong commitment to delivering exceptional quality patient care to JOIN OUR TEAM. If you enjoy case management and possess excellent clinical nursing skills consider working with us. As a clinical case manager, you will be responsible for completing a detailed initial assessment of the patient, the family, and the facility/home environment to determine specific care needs. You would be responsible to continue to monitor on a weekly basis as they continue their end-of-life journey.
Job Type: Full-time
Pay: $85,000.00 - $95,000.00 per year
