Hospital/Forensic Intensive Case Manager (Full-Time)

Want to work for an agency that has been voted Best Places to Work in Western PA for 4 Consecutive years? CCR is currently seeking a professional and self-motivated MH Hospital/Forensic Case Manager to serve Clarion County individuals.

Essential Functions:
  • Coordinate with local MH hospital staff, State Hospital staff, Long-Term Structured Residence (LTSR), local psychiatric facilities, and the County Correctional facility to ensure collaborative planning and timely connection to services for persons living with mental illness and involved with law enforcement.
  • Complete all reports, documentation and data entry in a timely and accurate fashion.
  • Serve as the Mental Health professional point person who will locate, coordinate and monitor services for individuals with forensic involvement from Clarion County at local, state and regional facilities.
  • Maintain accurate records, documentation and utilization requirements
  • Complete and process timely intakes for consumers accessing the base service unit and ensure referrals are made.
  • Communicate effectively with direct supervisor and co-workers. Be able to work as a team with similar case managers across 9 counties, and with court officials, psychiatrists and staff affiliated with the Long Term Structured Residential program located out of county.
Responsibilities:
This position will provide a highly specialized forensic/mental health case management service for persons who are incarcerated or under the supervision of probation or parole. This position will act as liaison on behalf of an individual who is seeking mental health supports to community based services , legal entities, courts, and family.

Qualifications:

Bachelor Degree in Human Services or related field and one year mental health experience, one year case management experience and forensic experience. ACT 33/34 and FBI Clearances, valid driver’s license and proof of car insurance coverage must be provided prior to hire. Must have computer competency skills to learn agency software. Must maintain strict confidentiality and be able to exercise sound judgement, be well organized and follow precise timelines. Individuals who have accessed human services are encouraged to apply.
