Abode Services, :one of the largest and effective nonprofits working to end homelessness in the Bay Area, is seeking a Housing Case Manager :for Santa Clara County.
SUMMARY:
The Housing Case Manager is responsible for the provision of housing stabilization services to individuals or families who are at risk of homelessness. This will include conducting assessments, and intakes for households seeking assistance, processing applications, and providing ongoing case management support to increase housing stability. This position is also responsible for service coordination to ensure that participants are connected to other necessary supportive services. Benefits and Perks:

How You Make an Impact:
- Coordination of services for program participants to ensure that participants are receiving essential services to increase housing stability.
- Processing requests for housing related financial assistance, including assessments, program enrollment documentation and check requests.
- Creates a housing stability plan, with participant, that includes housing, income, and other goals to help move participants to self-sufficiency.
- Provide case management services targeting maintaining housing.
- Meet with participants regularly to assess progress towards housing stability goals
- Attend program meetings with internal and external partners to coordinate services and ensure quality services.
- Responsible for complete documentation as required by all program components. Case files need to be complete, accurate, and organized according to Abode standards. Files/cases should be closed in accordance to program standards.
How You Meet the Qualifications:
- 2-3 years of experience of case management and strong knowledge of community resources.
- High School Diploma/GED required. AA preferred
- Reliable transportation along with a valid and current CA Driver's License and insurance is required.
