% Time Role Description
75% Case Management:
1. Provide Case Management services to HMI youth members. Ensure youth enrollment in program and assertively shepherd youth through contracted services including referrals and linkage to care.
- Provide necessary information and referrals for housing, medical, vocation, and legal assistance as well as entitlement and identification information to clients.
- Accompany youth to off-site offices to provide advocacy and linkage to care, as needed.
- Assist other staff in the process of assessing youth needs for case management services, and doing outreach/in-reach to engage new youth in services.
- Maintain linkages with agencies that can provide case management assistance programs on-site at HMI (such as Medicaid enrollers and legal services clinics).
- Conduct follow-up to confirm referrals kept and access to services.
2. In collaboration with the General Case Manager, create individualized service plans with homeless youth to address housing and shelter needs.
3. Support youth members in completing NY Supportive Housing Applications and/or helping youth navigate the housing voucher system as part of assisting eligible homeless youth with placement in supportive housing.
4. Function as entitlements advocate.
5. Support program outreach staff in conducting community outreach events as needed including street outreach, piers, Pride events, shelters, parks, day centers, youth service fairs, etc.
6. Supports counseling staff with counselor on-call and crisis counseling and new member intake services as needed.
7. Work with supervisor to design and implement group level activities to support youth engagement in Case Management.
8. Participate in supervision, team meetings, internal/external meetings for staff development and support to clients.
9. Manage collaborative relationships between departments and outside agencies.
10. Complete all required documentation and data input, including monthly and quarterly reporting in compliance with contract requirements.
11. Maintain all electronic charting requirements in Salesforce for comprehensive client files.
25% Programmatic Direct Service:
- Assist in the facilitation and coordination of groups, activities, events and other programing and direct services to youth to support the HMI Milieu-Membership model.
- Work closely with other HMI departments and staff to support ongoing programs and services.
- Outreach and engage youth during Program Common Hours (3:15pm - 7:15pm).
- Prepare for and facilitate weekly group-based activities for general programming.
- Assist in the facilitation of drop-in groups and programming.
- Perform general staff duties, such as Café duty, train-pass, general event set- up/breakdown, etc.
- Participate and assist with general HMI agency-wide events within and outside of HMI.
As needed: Other duties as assigned by supervisor.
Knowledge, Skills and Abilities Required:
- Bachelor's degree in health education, social work, creative arts therapy, public health, or related human services field.
- At least two years' experience working with a diverse population of adolescents in an urban setting, including knowledge of supports needed for LGBTQIA+ youth from various cultures and backgrounds.
- Knowledge of liberation-focused healing and anti-oppressive methods; able to describe how they support and uplift BIPOC and TGNC individuals and communities in their work.
- Experience facilitating group discussions and managing a variety of small and large group dynamics.
- Experience planning and implementing health-specific programs.
- Excellent verbal and written communication skills, including conflict mediation and public speaking.
- Knowledge of harm reduction practices, trauma-informed care, and positive youth development models.
- Ability to exercise independent judgment when appropriate; strong decision -making skills.
Technology Skills: Strong with proficiency in Zoom, Microsoft Word, Excel, PowerPoint and Outlook.
Additional Desirable Qualifications:
- Bi-lingual (Spanish-English) skills preferred.
- Master's degree and/or licensure in Social Work, Creative Arts Therapy or Mental Health Counseling preferred.
- Extensive knowledge of one or all of the following health and wellness related issues: HIV, Sexual and Reproductive Health, Pregnancy Prevention, substance use, nutrition, and violence prevention.
Working Conditions:
- Hybrid schedule of virtual and in-person work. In person work primarily takes place at 2 Astor Place, NY, NY 10003, except for outreach work.
- Program work schedule: Monday-Friday: 12:00 pm to 8:00 pm.
