Description and Responsibilities:
Adult Community Clinical Services (ACCS) are comprehensive, clinically focused services provided by a multi-disciplinary team that provides services to adults with mental health needs. Our Housing Coordinators play an important part of this multi-disciplined team and we are looking for energetic, caring, mission focused individuals to fill these roles!
The Housing Coordinator will:
- Embrace and Operate within the Housing First Principles in order to set a foundation for pursuing health and recovery goals with people served.
- Collaborate with the ACCS Integrated Team to establish treatment goals, determine housing and service needs, and eliminate barriers to housing.
- Assist with site visits, application and lease negotiations. Assist individuals with lease application(s).
- Collaborate with landlord to ensure apartment is ready for occupancy, perform move-in/out inspection with person served and conduct ongoing safety and cleanliness inspections.
- Support individuals as the move, set up, and maintain the household.
- Provide education and support to assist individuals in meeting all lease terms, subsidy requirements and related benefits.
- Assist individuals served with monitoring lease renewals, maintenance issues, and housing appeal process as needed.
- Orient individuals to the community, neighborhood and available resources including transportation.
- Provide follow-up home visits to review progress towards self-sufficiency including support, advocacy, reduction of isolation, listening, problem solving, and identification of resources to assist with integration of Persons served in their new community.
- High School Diploma or equivalent. Alternative education/experience in lieu of formal education will be considered.
- Valid Driver’s License and acceptable driving record.
- Reliability, willingness to learn, and being open to new opportunities.
