Community Youth Services is recruiting for Care Coordinator. The Transitional Age Youth (TAY) Care Coordinator is a vital part of a treatment team that provides home, family, school and community-based intensive Care Coordination services to young people between the ages of 15 years and 21 years in Thurston and Mason counties. They work within a team, and within multiple systems, to support youth and families in increasing tools and strategies to navigate systemic involvement as the youth prepares to drive their own treatment.


The Care Coordinator provides individualized services and will offer a flexible schedule to meet the youth where they are at, this may include some appointments on nights and weekends. The TAY Wrap-around Facilitator will be trained in Wraparound with Intensive Services (WISe), Positive Youth Development, MAP (Managing and Adaptive Practices) + WRAP (a modularized evidence-based practice), as well as safety planning and CANS assessments. The Care Coordinator participates in a cohesive compassionate team to bridge gaps in communication and meet the needs of the youth served while building natural supports, promoting healthy boundaries, encouraging positive productive change, and inspiring hope. CYS is currently providing telehealth and in-person services to support youth/young adult needs.


Starting pay is $22.19 per hour. (Non-exempt) (Full-time).

Position Qualifications:

  • Bachelor’s Degree in Social Work, Psychology, Counseling or related field or Associate’s Degree with two years experience in cross-systems care coordination or case management.
  • Requires the following Washington State credentials: Agency Affiliated Counselor
  • Experience working within multiple systems including: Mental Health, Juvenile Justice, Schools, the Department of Children, Youth, and Families (DCYF) Department of Social and Health Services (DSHS), Department of Vocational Rehabilitation (DVR) community and natural supports for families, etc.,
  • Experience and/or knowledge of cultural issues related to marginalized youth and young adults and alignment with the prinicipals of Positive Youth Development and Wraparound. Able to maintain a non-judgmental attitude,
  • Experience with and understanding of the legal system related to children and families including: dependency orders, juvenile court procedures, and DCYF procedures,
  • Must possess a high level of self-initiation, efficiency, organization skills and the ability to meet mandatory deadlines,
  • Proficiency in facilitating meetings, presenting information to groups, coordinating tasks or teams, leadership, and developing working relationships with others from diverse populations and/or with different roles,
  • Proficiency with Microsoft Office with the technical skills to efficiently use MS Word and Outlook, the internet and the intranet is also required, and
  • Ability to work evenings and weekends to meet the needs of youth and families where and when they need support.

Agency Qualifications:

  • The ability to pass an extensive background check with no history of conviction or perpetration of child abuse or physical harm to a child or children or an individual aged 18 years and above who is or may be unable to take care of themselves or is unable to protect themselves against harm or exploitation by reason of age, illness, trauma or disability, or any other reason.
  • Professional and personal conduct that maintains a personal and professional background so that criminal history remains free from unlawful conduct. Certain crimes may disqualify employees to have unrestricted access to children or vulnerable populations in accordance with Department of Social Health and Services policy and Washington State law. Professional or personal conduct that causes this clearance to be suspended or denied will result in suspension and/or termination of employment. (Pursuant to RCW 43.43.832, 43.43.832(7), 43.43.834, 43.20A.710 and WAC 388-06.).
  • A valid Washington State Driver License with a minimum of five years’ driving experience and a three-year good driving record evidenced on a Washington driver’s abstract. The ability to pass and maintain a clean driving records check at employment and annually. Professional or personal conduct that results in a restricted or denied driving status could result in termination of employment. (Candidates relocating to the area must provide current proof of three-year driving record from current state; and have ten days from date of employment to provide a copy of a Washington Driver’s License and Washington driver’s abstract.).
  • Proof of personal auto insurance levels provided on your insurance declaration page. Coverage must be current, and copies provided to HR at renewal. Coverages at or above $100,000 combined single limit is required for positions that require driving and is recommended and preferred for positions that have occasional driving.
  • A sensitivity and responsiveness to the cultural differences in the organization's employment and service population.
  • Must possess or obtain within 30 days of employment CPR/1st Aid/Bloodborne Pathogen certification. Some positions require CPR before a first shift.
  • Strong interpersonal skills, including the ability to work as part of a team.

MAJOR RESPONIBILITIES AND RELATED TASKS:

Program Responsibilities

  • Provides home, school and community-based Care Coordination to youth and their natural supports following the principles of Wraparound to fidelity in a youth-driven way, guided by the identified needs and strengths of the youth, focused on outcomes and incorporating the tools of “Managing and Adapting Practices (MAP+WRAP)”. Acts as central point of communication for the WISe team.
  • Maintains an average caseload of 10-15 participants.
  • Coordinate and Facilitates Wraparound Team Meetings:
  • Develops a meeting agenda with the youth, natural supports, and other wrap team members focused on outcomes by meeting needs and building strengths. Prepare visual aids or tools to facilitate the meeting process.
  • Schedules meetings at a place/time that is accommodating (comfortable and convenient) to the youth and natural supports and available to all wrap team members and inviting all team members. Contact wrap team members who are unable to attend a meeting, in advance, to elicit their input.
  • Engages all wraparound team members and identify their needs for meeting agency mandates. The Care Coordinator is expected to identify the strengths and needs of the youth, family, and team, provide wraparound team members with an overview of wraparound team practice, and clarify their role and responsibilities as a team member in this process.
  • Maintains team focus on the scope of work for the WISe team and progress/movement toward transition. Regularly acknowledge and celebrate successes and transitions.
  • Ensures all wrap team members receive an updated copy of the cross-systems care plan, documentation of progress, wrap team meeting activities, discussions and task assignments after each wrap team meeting.
  • Establishes and sustains an effective team culture and an emotionally-safe, youth-driven process. Demonstrates ability to show cultural humility and provide a non-judgmental space for youth, families, and professionals.
  • Shows ability to maintain a stance of appreciation and acceptance of youth and families, including their choices.
  • Demonstrates leadership experience and diplomacy in resolving conflicts and integrating divergent perspectives.
  • Maintains mandated data on each family as prescribed by 'Managing and Adapting Practices,' the Thurston Mason Regional Support Network and the University of Washington Evidence-Based Practice Institute.
  • Participates in meetings, supervision and consultation as required. Maintains flexible availability to participate in internal team consultations, as well as scheduling with participants and families.
  • Increases the ‘natural supports’ in wraparound team membership and the youth/family’s integration into their community. This is accomplished by getting to know the family history, culture, and resources, and by helping the family to identify and engage potential supports. Examples of natural supports include friends, extended family, neighbors, members of the family’s faith community, co-workers. The goal is to have more natural and informal supports on the team than formal supports.
  • Provides regular, and ongoing communication and collaboration with each participant’s Wraparound team and community partners, providing information on the modalities we use to obain stabilization and positive outcomes for the youth we serve.
  • Shares resources and information in an individualized manner so that youth and families understand the WISe process and have access to information regarding their care.

  • Utilizes consensus-building, conflict management techniques to meet the needs of the youth and their supports to create a single cross-systems care plan. Encourage each wraparound team member to identify their priority concerns, work proactively to minimize areas of potential conflict, and acknowledge the mandates of others involved in child-serving systems.
  • Maintains documentation as prescribed by ‘Managing and Adapting Practices,’, the Thurston Mason Regional Support Network and the University of Washington Evidence Based Practice Institute, and
  • Assists participant’s Wraparound team in developing a sustainability plans as well as community and natural supports that will provide ongoing support when formalized TAY services have ended.
  • Works with the Youth and Family Peer Specialists to identify peer support, family support, or other system and community resources that can assist the youth and family with exercising their voice in the wraparound process.
  • Utilizes agency Electronic Health Record system (EHR) to document services provided in a timely manner. Engages with a data-driven model to monitor participant needs, outcomes, and successes.

Professional Development:

  • Attends required staff meetings and contribute input on program and agency issues.
  • Attend scheduled in-service training for on-going development of professional skills.
  • Meets with supervisor on a monthly basis to review performance and SMART goals.

Other Related Responsibilities

  • Attends relevant trainings, conferences and department/ program staff meetings.
