Infant Intake Coordinator Job at District 19 Community Services Board
District 19 Infant Intervention Program is seeking a qualified professional to assume the responsibilities of Infant Intake Coordinator. The Intake Coordinator is responsible for accepting referrals to the early intervention system, opening the referral to the agency and Part C, assessing family priorities/concerns/resources as well as gathering information regarding the child's development and functioning to determine eligibility for early intervention services.
Also responsible for scheduling intake appointments and initial Assessment for Service Planning/Individualized Family Service Plan meetings, providing orientation to the CSB and to Early Intervention, reviewing family rights and safeguards, and accurately documenting all services in the appropriate system, in a timely manner.
District 19 Infant Intervention Program services the communities of Colonial Heights, Hopewell, Prince George, Petersburg, Dinwiddie, Sussex, Surry, Emporia, and Greensville County. On the job travel within the catchment area is required.
Successful applicant must have a minimum of a Bachelor’s degree in rehabilitation counseling, recreation therapy, occupational therapy, physical therapy, speech/language therapy, child and family studies, counseling, early childhood, early childhood growth and development, early childhood special education, human development, human services, music therapy nursing, psychology, public health, special education- hearing impairments, special education-visual impairments, or other related field or interdisciplinary studies approved by the DBHDS.
Must have knowledge of child development from birth to 5 years, community resources related to service delivery, and support services available to families; demonstrate effective written, oral, and interpersonal communication principles and techniques; skills in building rapport with families and team members, including communication, listening, and problem solving with families from diverse socio-economic backgrounds; organization and ability to complete multiple tasks at same time. Applicant must demonstrate time management and organizational skills.
Applicant must meet discipline-specific qualifications and complete four required training modules for Early Intervention certification within one month of hire.
Experience providing intakes, evaluations, or assessments with infants, toddlers, and/or youth; knowledge of service delivery to young children, including coaching, to infants/toddlers with developmental delays; ability to coordinate and facilitate multidisciplinary team activities; skills to include coordinating and monitoring services provided to infants and families and basic computer skills; skills in conflict resolution; knowledge of general principles of client record documentation and general knowledge of the Individual with Disabilities Education Act (IDEA)/Part C; ability to identify and coordinate community activities to support the family's participation and inclusion.
A bilingual (English/Spanish) candidate is preferred.
Starting salary is $44,771
