The Intake Care Coordinator's primary responsibility is to handle in-coming inquiry calls and information facilitating placement with the appropriate RVNAhealth business line.

Responsibilities:

  • Accepts referrals for care from any source; obtains accurate data complete as possible.
  • Sets up episode for care in computer system.
  • Informs Physicians of referral acceptance and obtains agreement for MD to follow in community.
  • Provides information regarding all inquiries and referrals to Manager for referral log.
  • Corresponds with clinical field staff via electronic medical record to communicate details of referrals and report any relevant information.
  • Identifies problems and potential problem situations as they become evident
  • Develops an in-depth knowledge of Medicare, insurance and Title 19 payer requirements.
  • Coordinates requirements for payers such as authorization, required documentation or contacts.
  • Covers weekend time as needed including intake and scheduler positions.
  • Assist as a back-up or support to other roles as time allow/as needed.
  • Asks for help when needed.
  • Participates in total quality improvement process for both agency and self.
  • Accepts and utilizes supervision constructively in order to enhance the quality of agency documentation and professional self-development.

Qualifications:

  • Some clinical experience, including medical terminology.
  • Ability to handle several situations simultaneously in a professional, calm, dependable manner.
  • Ability to prioritize multiple tasks in an organized manner.
  • Must be detail oriented.
  • Exceptional customer service skills including phone manner and ability to build relationships in a fast paced environment.
  • Is computer literate, enters information into computer in a timely and accurate manner.
  • Knowledgeable of sales theories and practices.

Note: This will primarily be a Mon-Fri position, however weekend coverage may be requested occasionally.
