INTAKE REGISTERED NURSE Job at Girling Health Care of New York, Inc.
Candidate must be a Nurse. Nurse would work in our Brooklyn office facilitating referrals to Home Care by gathering and interpreting medical and financial data, speaking with patients and families and working with referral sources to ensure optimal transition to Home Care.
Qualifications
1. NYS Nursing License
2. Two years of experience working in an acute care setting; one year of home care and/or hospice experience preferred.
3. Working knowledge of community resources.
4. Able to meet, collaborate and work efficiently with others in the community.
5. Must be proficient in the use of Microsoft programs for internal and external communications, presentations, database tracking/reporting, and spreadsheet applications.
6. Must have excellent oral communication skills.
7. Is self-directed and possesses the ability to work with little supervision.
