Job Description

Position: Family Interventionist

Program: Lehigh Valley Programs

Reports To: Program Supervisor

Employees Supervised: None

Effective Date: 11/27/16

FLSA Status: Non-Exempt

Purpose:

To provide quality family services as part of the community -based service team.

Requirements:

1. Masters degree (preferred) in social work, psychology, counseling or other related field.

2. One (1) to three (3) years experience working with youth in a Human Service field.

3. At least three (3) written references that demonstrate past educational and/or employment experience.

4. Successful completion of core development orientation program.

5. Must be at least 21 years of age.

6. Valid PA driver’s license (of ability to obtain such license), and good driving record, with proof of auto insurance coverage.

7. PA Act 33/34 and 151 clearances.

8. Pre-employment physical from a licensed physician (after conditional job offer) verifying person has no communicable diseases or physical conditions that might endanger the health and safety of children or prevent the person from performing essential job functions.

Job Duties: * Essential Functions

1. DEMONSTRATES ABILITY IN THE FOLLOWING CORE AND PROGRAM COMPETENCIES:

A. Effective use of verbal and non-verbal communication skills with adolescents/families.*

B. Ability to develop and maintain effective and relationships with adolescents/families.*

C. A strong familiarity with various community resources.

D. Therapeutic crisis prevention and intervention; First Aid and CPR; infectious disease control policies and procedures; emergency fire procedures/evacuation; Emergency medical procedures; Child Protective Services Law; Child abuse recognition and staff responsibility; Vehicle safety.*

E. Ability to assess, interpret and effectively document data specifically related to the care of adolescents.

F. Working knowledge of various intervention strategies.

G. Telephone system and proper telephone etiquette.

H. Use of general office machines: copy machine, fax machine.

I. Unit/program and agency policies and procedures.

J. Continuous Quality Improvement (CQI): theory and processes.

K. Demonstrates knowledge in the safe use of equipment according to manufacturers’ recommendations and removes malfunctioning equipment from use; labels and reports problems appropriately and in a timely manner.

2. ADMINISTERS QUALITY FAMILY SERVICE PROVISION TO ASSIGNED YOUTH AND FAMILIES:

A. Maintains confidentiality of each client and their family.*

B. Provide services to clients and families as assigned.*

C. Provides individual, goal-oriented service for clients as determined by the team.*

D. Attends all internal and external meetings as assigned.*

E. Provides structured groups for adolescents/families in coordination with the interventionists.*

F. Completes all required documentation including service plans/reviews and discharge summaries.*

G. As part of a team, engages in preventative crisis intervention techniques and intervenes in crisis situations, using only the methods/techniques that are approved and taught by The Children’s Home of Reading.*

H. Does not befriend a child to the disregard of the professional role, and maintains appropriate working boundaries.*

I. Supports goals and objectives for each client.*

J. Uses time in a constructive and positive manner (i.e. review files, training materials, 1:1 time with children, etc.)*

K. Performs on-call responsibilities as assigned.*

L. Assists in training or orienting new interventionists or interns as assigned.

M. Adheres to Universal Precautions and agency policies and procedures relating to infectious Disease Control.*

3. PARTICIPATES IN QUALITY ASSESSMENT AND IMPROVEMENT AND OTHER ACTIVITIES USING ESTABLISHED CRITERIA TO

ASSURE THE QUALITY AND APPROPRIATENESS OF SERVICE:

A. Assesses and identifies department problems/needs and opportunity for improvement.

B. Devises solutions to identified department problems in cooperation with management.

C. Meets deadlines for assigned projects.

D. Performance reflects use of Quality Assessment and Improvement results to improve personal practice.

4. DEMONSTRATES EFFECTIVE VERBAL AND WRITTEN COMMUNICATION AND INTERPERSONAL SKILLS AND THE ABILITY TO

WORK SUCCESSFULLY AS A TEAM:

A. Documents in a complete, clear and concise manner.

B. Demonstrates knowledge of community-based programs and organizational structure and follows proper lines of communication.

C. Uses verbal, non-verbal and written communication skills, which encourages and fosters positive intra and interdepartmental relationships.

D. Reports unfavorable or unexpected changes in a client’s status to the appropriate team members in a timely manner.

E. Promotes good interpersonal relationships with professional disciplines, clients and all significant others.

F. Is courteous to clients, families and peers.

G. Handles telephone calls and inquiries with courtesy, accuracy, and respect for confidentiality.

H. Works harmoniously with other team members and accepts/acts on directives and responsibilities given by supervisor.

I. Communicates pertinent information with co-workers as well as support staff via oral or written methods, which includes notes, incident reports, etc.

J. Maintains a team consistency with clients.

K. Assists in the development of staff by responding to team members in a supportive manner.

L. Maintains professional relationships within the workplace.

5. DEMONSTRATES ACCOUNTABILITY AND RESPONSIBILITY FOR OWN PROFESSIONAL AND PERSONAL GROWTH :

A. Actively participates in team meetings, program meetings and all employee meetings.

B. Attends mandatory in-service programs or meetings.

C. Successfully completes all annual core training renewals. Attends at least 40 hours of training annually.

D. Assumes the role of child advocate.

E. Demonstrates flexibility in response to unexpected changes in workload and performs additional duties as assigned.

F. Demonstrates a positive and dependable attitude in relation to work assignments.

G. Keeps informed and accountable for reviewing current unit minutes, memos, pertinent posted information and required reading materials as evidenced by documentation.

H. Demonstrates an ability to adapt to changes in program functions, management styles and unit standards.

I. Demonstrates initiative in work performance as evidenced by: volunteering for additional work assignments or committees, autonomously completing tasks or projects that would increase the quality of services or job efficiency; providing useful solutions to problems to supervisors or management, etc.

J. Completes staff surveys, training evaluations, questionnaires, etc. as required.
