Who We Are: Hill Country Health and Wellness Center is a Federally Qualified Health Center providing integrated primary care, mental health care, substance use disorder treatment, dental care and other community programs and services aimed at improving the health and well-being of people living in our community. We are a mission-driven agency that was started locally in the 1980s by a small group of young friends with a simple commitment to helping people in their community live healthier, more fulfilling lives. Over the years these efforts have led to the development of a variety of direct services and programs. Presently, our main clinic is in Round Mountain with several small satellite clinics located in Redding, plus an urgent mental health resource center, a field-based Mobile Crisis Outreach Team, and Wellness Centers in Round Mountain and Burney that serve adults and teens. We also have several “complex care” teams working in Redding and in eastern Shasta County. These teams include therapists, case managers, and nurses who work with primary care and psychiatry to provide intensive outpatient care for persons with chronic diseases, serious mental health problems, addictions, and other life challenges. Hill Country is best known for being genuinely caring and kind, going the extra mile for our clients, and for working with others to create positive change in our community.
Job Description: Therapists at Hill Country typically see 5 to 7 clients per day, with support and camaraderie from other experienced therapists and staff being available at any time. When therapists work collaboratively with Primary Health Care Providers, Case Managers, and other team members to create and implement comprehensive care plans from a whole person perspective. This integrated care approach provides plentiful opportunities for creative problem solving, consultation, and other support, while also resulting in higher client satisfaction, better health outcomes, and broader social impact.
Responsibilities generally include:
- Engage with and build effective helping relationships with a wide variety of clients and, whenever appropriate, their families and other significant persons
- Complete comprehensive psychosocial assessments
- Develop individualized psychotherapy goals and intervention strategies, often as part of a broader team-based care plan
- Communicate and collaborate with others to ensure optimal care (e.g., clients, caregivers, primary care, psychiatric specialty care, case managers, and other involved persons)
- Provide ongoing psychotherapy interventions and facilitate transitions in care, as appropriate
- Provide crisis prevention, intervention, stabilization, and recovery support, as needed
- Arrange for assistance with care coordination, as needed
- Refer to appropriate community resources and other services, as needed
- Document all services provided in the client’s confidential electronic health record
- Participate in group case discussions and training and offer consultation to others
- Work creatively with others to develop and improve Hill Country’s services and resources
