Lead Clinician Erie Teen Center Job at Erie Family Health Center
GENERAL SUMMARY
- Serves as the lead clinician at Erie’s Teen/Young Adult site. Works with Site Operations management as well as adolescent leaders across Erie to provide high quality health care and programming for this age group. May also provide some care at Erie’s school-based or other sites as assigned.
- General Erie clinician responsibilities include:
- Provide comprehensive primary health care service to patients in the prevention, diagnosis, and treatment of disease.
- Acts as primary health care provider with the scope of his/her primary care specialty.
- Performs physical examinations on new and return clients. Refers patients to specialists, as necessary.
- Documents all patient care activities.
- Maintains a productivity level necessary to reach mandated annual level of encounters.
PRINCIPAL DUTIES AND RESPONSIBILITIES
- Works with Adolescent team leadership at the Teen Center and across Erie to develop and implement best practice models of Adolescent/Young Adult care
- Works with site leadership to optimize patient care at the Teen Center
- Performs history and physical on new/return patients throughout the lifespan.
- Orders appropriate diagnostic/screening test and reviews test to establish diagnosis; initiates or continues treatment based on findings.
- Manages acute and chronic illness according to established protocols. If appropriate, works with collaborating physician.
- Provides follow-up health care providers or community agencies, as necessary.
- Maintains a productivity level necessary to reach mandated annual level of client encounters.
- Keeps accurate records as required.
- Participates/leads appropriate interdisciplinary teams. Contributes toward development of annual action plan health care plan as appropriate.
- Initiates and/or participates in on-going QI activities at Erie Family Health Center.
- APP - Is responsible for Continuing Education Units to maintain safe level of practice and certification. MD/DO – completes appropriate Continuing Medical Education for licensure and for maintenance of board certification
- Participates in the development of clinical protocols and health center policy/procedure development as appropriate.
- May perform procedures such as colposcopy, Norplant insertion/removal, IUD insertion/removal, cryotherapy etc. with appropriate training and privileging (see credentialing by specialty).
- Precepts learners as appropriate for specialty
- Participates in annual performance appraisal/peer review.
- Interprets the philosophy of EFHC to other staff and clients.
- Represents EFHC externally as assigned.
- Maintains an active collaboration practice agreement with an Erie physician if required.
- Adheres to all Erie policies & procedures.
- Performs other duties as assigned.
- Key Result Areas:
- Direct patient care
- Dedicated time each week to work on adolescent initiatives/site issues
QUALIFICATIONS
- M.S. in Physician Assistant Studies, MSN, or DNP from accredited nursing school
- Active PA or APRN license in Illinois
OR
- MD or DO from accredited institution, licensed physician in Illinois
- Board Eligible or Certified in Family Medicine, Pediatrics, OB-Gyn
For all:
- Active DEA and controlled substance License
- Active CPR Certification
- 2 years of experience in an outpatient setting as an APP/MD/DO strongly preferred
- Demonstrated interested in health care for Adolescents required
- Previous healthcare leadership experience strongly preferred
- Previous experience in FQHC, Community Health Centers or similar strongly preferred
