· Assist in the implementation of admission, discharge and transfer of residents according to facility procedures.

· Resident Rights

· Know the resident’s rights. Help the residents exercise and/or protect their rights

· Become knowledgeable about resident’s choices and follow those choices accordingly.

· Report resident’s concerns to the supervisor

· Maintain confidentiality of resident information.

· Provide psychological support to all residents to help them adjust to their disabilities.

· Preserve the dignity and comfort of the resident.

· Assist in managing scheduling

· Assist and encourage residents to be independent in areas of activities of daily living.

· Assist and encourage residents to be active for reasonable periods of time as permitted.

· Promote resident mobility through proper transfer techniques to/from bed, wheelchair, and stretcher.

· Assist with ambulating as needed. Assist with daily range of motion exercises.

· Maintain good body alignment and proper positioning of residents.

· Transport residents from one place to another.

· Elimination – (Monitor elimination status of residents.)

· Offer and remove bedpans and urinals.

· Assist residents to use commode.

· Instruct and assist with bowel and bladder program

· Check, change and clean incontinent residents as instructed

· Document and report bowel and bladder patterns.

· Collect urine and stool specimens as requested.

· Give catheter care: measure and record intake and output.

· Mealtimes – (Assist at mealtimes)

· Prepare residents for meals. Assist to dining room as necessary.

· Feed or assist residents.

· Instruct residents in use of self – help devices.

· Serve nourishments.

· Observe residents for difficulties in chewing or swallowing while eating as needed.

· Vital Signs

· Take, record and report vital signs and weights using a variety of sphygmomanometers,

thermometers and scales.

· Infection Control

· Practice proper hand washing technique.

· Handle clean and dirty linens properly, without contamination.

· Follow standard precaution procedures: use protective equipment and properly dispose of

contaminated wastes.

· Environment and Safety (Assure a safe, clean and comfortable environment for resident)

· Place call lights within reach of residents and answer call lights promptly.

· Know and implement facility safety rules.

· Demonstrate proper use of equipment. Report equipment needs or repairs immediately

· Clean equipment and utility areas as assigned.

· Assure adherence to smoking policy.

· Observe proper ventilation, temperature, light and noise level for day and night activities.

· Report any incidents or accidents to the RCS or Supervisor

· Keep the resident living areas neat and orderly: personal care items stored properly;

clothing hung in closets or placed in soiled bins as appropriate.

· Assist in fire control and resident evacuation during fire or disaster emergencies.

· CPR

· After receiving CPR training, must be able to be a first responder in a medical emergency;

e.g. initiate and continue CPR until emergency help arrives.

· Observations and Reporting

· Report at beginning and end of each shift of duty.

· Perform and document resident care activities according to written and verbal instruction

from the RCS or supervisor.

· Coordinate work assignment with other resident activities such as therapy, doctor

appointments, activities, etc.

· Observe resident for changes in behavior or condition and report all pertinent information

to the RCS or supervisor.

· Service Plans

· Participate in the development and implementation of resident service plans according to

facility policy and procedure. Keep informed of revisions in care plans through

participation in mini-conferences.

· Hospitality and Dignity

· Maintain a friendly, helpful attitude toward residents, their families, staff, volunteers and

visitors.

· Attendance

· Maintain a consistent attendance record to ensure proper resident care.

· Orientation and Inservice Education

· Participate in required orientation and inservice programs and attend staff meetings as

requested

· Policies and Procedures

· Adherence to all facility policies and procedures.

Job Type: Full-time

Pay: $20.00 - $22.00 per hour
