Case Managers make a real impact on the lives of individuals with developmental disabilities. The Moore Center provides a wide range of services to help “Create opportunities for a good life” for the individuals we proudly serve.

We are seeking a full-time Level One Case Manager to provide administrative support to help ensure compliance with Bureau of Developmental Services regulations through the completion of core case management tasks. You’ll be an integral part of our Case Management team and be responsible for preparing, facilitating and writing of service agreements, assisting with the audit process and preparation, reviewing client monthly progress notes, completing monthly contacts with a member of the client service team and case notes, assisting with client face-to-face contacts, Supports Intensity Scale referrals and filing, along with other regulated case management tasks.

Qualifications:

· Bachelor’s degree preferred; consideration may be given to those with an Associate’s degree and a minimum of two years of work experience in human services

· 1-2 years of experience working in human services preferred, but not required!

· Proficiency in Microsoft Office Suite and database systems

· Valid driver’s license and access to a safe operating vehicle during scheduled working hours

*Guidelines:

· Sign-on Bonus is effective until Dec. 1, 2022

· The bonus is effective for all eligible employment applications and resumes

· Bonus pay-out will occur after employee completes a favorable 30-day probationary period

Job Type: Full-time

Pay: $18.25 per hour
