This professional is an integral member of the patient and families care team and provides emotional support, counseling, goals of care conversation and assists in coordination of care for patient and family with varied healthcare teams.
This is a home health position covering the Danbury, Redding, Ridgefield and other areas nearby!
Responsibilities:
·Perform Initial Hospice Social Work Comprehensive Assessment within 5 days of NOE
·Conducts ongoing reassessments of patient/family needs and counseling as required.
·Provides short term crisis intervention and individual or family counseling when indicated.
·Provides services, under the direction of a physician (who approves plan of care). Referral to Community resources as appropriate.
·Observes, assesses and brings to IDG conferences information regarding psychosocial, emotional, spiritual, physical and financial conditions affecting the patient and family. Assist other IDG in understanding the significant social and emotional factors related to client's health problems.
·Assumes the active role of advocate for the client/family and caregiving unit.
·Provides ongoing counseling related to issues of death and dying to the client, family and caregivers as needed.
·Attends staff meetings, IDG, case conferences, educational programs, and other meetings as assigned and appropriate and assigned. Attend peer consultation meetings as mandated by the State of Connecticut.
Qualifications:
·Master's degree in social work from a school accredited by the Council on Social Work Education.
·LCSW Required
· At least five years of social work experience, after master's degree has been obtained.
· One year of experience in Hospice & Palliative care setting preferred. One year in health care setting required.
·Experience and ability in end of life client, bereavement and family assessment and care planning including but not limited to Advance Directive/Living Will/POA Care Planning, End of Life physical, emotional, and spiritual symptom control, and Client and caregiver advocacy and support skills.
·Excellent communication skills and ability to work successfully with others during times of increased stress
·Ability to work independently, make accurate, and at times, quick judgments. Ability to respond appropriately to crisis independently.
· Acceptance of and adaptability to different social, racial, cultural and religious modes.
·Is computer literate and able to document at point of care.
·Is able to drive and travel in all types of weather and environmental conditions.
