FAIRHOPE Hospice & Palliative Care, Home of The Pickering House is seeking a Licensed Practical Nurse for its Palliative Care program to work with patients, families and other professionals to promote the philosophy of palliative care and the hospice concept. Our LPNs have a pivotal role in the palliative goals and daily care of our patients. FAIRHOPE looks to its LPNs to apply appropriate nursing care to foster the best possible experience for our palliative patients, their loved ones and care givers.

We believe in a professional team approach. This position works in collaboration with the Palliative Care Nurse Practitioners and Palliative Care Coordinator to organize admission information such as history and physical data, referrals, patient visits, physician orders and other responsibilities.

Candidates must demonstrate great communication and social interaction skills, as well as, be a compassionate individual to work with other professionals and patients and families who face a vulnerable time in their lives. The ability to analyze and implement proper action is essential.

Candidates must be a graduate of an accredited practical nurse or vocational nursing program and be currently licensed as a Licensed Practical Nurse in the State of Ohio. Possess and maintain CPR certification. Have three years nursing experience. Community health/hospice or medical/surgical experience preferred. Computer skills required.

This position is full-time, 35-40 hours/week (evening and weekend hours availability as needed). *New competitive wages*

Job Description Overview:

  • Ensures quality and safe delivery of Palliative services to patients/families/caregivers.

  • Implements current nursing practice following a patient assessment and an approved plan of care initiated by the Nurse Practitioner.

  • Provides accurate and timely documentation of patient services.
  • Provides patient/family/caregiver education and information pertinent to diagnosis and agency services.
  • Participates in transition from palliative to hospice services, appropriately reporting the need for Hospice to Intake Department.
  • Evaluates patient’s response to treatments/medications; reports to Nurse Practitioner as needed.
  • Completes pre-admission evaluation to appropriate program, i.e., hospice vs. palliative, data collection. Prepares chart/data entry.
  • Makes on-site visits to new referrals to set up palliative services. Follows up calls to admitted patients as needed.
  • Triage patient calls during normal business hours.
  • Follows up with patient, reporting change in condition to Nurse Practitioner.
  • Remain HIPAA compliant at all times.

Physical Setting: Hybrid position in various healthcare settings. Some driving is required.

Work Location: Service area coverage throughout Fairfield, Hocking, Perry and surrounding counties. Main office location is located in Lancaster, Ohio.
