Position Summary: The Charge Nurse Licensed Practice Nurse (LPN)/Registered Nurse (RN) is responsible for rendering nursing care in terms of individualized resident needs based on the scope of practical nursing, under the direction/delegation of the registered nurse and the residents’ attending physician. The RN/LPN will effectively interact with residents, family and other health team members while maintaining all standards of professional nursing.

Essential Functions:

  • Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.
  • Utilizes nursing knowledge and skills in the safe implementation of basic preventative therapeutic and rehabilitative nursing care of assigned resident as evidenced by documentation and observation of positive resident care outcomes
  • Performs delegated nursing functions using established procedures, policies, guidelines and standards as observed by the registered nurse.
  • Administers treatments, medications, and diets accurately medication accurately, observing patient response, as evidenced by documentation in the medical record and lack of negative outcomes
  • Admits and assess residents including initial care plan.
  • Maintains accurate narcotic count and sign in log book
  • Completes all required resident charting every shift.
  • Makes rounds on unit every shift; assess orderliness, infection control and resident care
  • Observes and documents changes in residents’ condition and report to the charge nurse, or attending physician and other departments.
  • Assist with directing, supervising, and assisting CNAs and other nursing staff in providing bedside care.
  • Works collaboratively with other members of the health care team in coordination of the resident’s care.
  • Performs other duties as assigned.

Supervisory Responsibilities: This position may have supervisory responsibilities related to CNA's. Work Environment: While performing the duties of this job, the employee is in skilled nursing facility atmosphere that has controlled heat and cold. The noise level is moderate to quiet. The LPN may be exposed to residents with contagious illness and must follow established protocols regarding infectious disease control.

Physical Demands:

  • While performing the duties of this job, the employee is regularly required to talk and hear.
  • This position requires a variety of activity levels including frequent standing, walking, bending, kneeling, stooping, crouching, occasional sitting, rarely crawling and climbing. The employee may be required to frequently lift items up to 50 lbs.
  • Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus.

Education / Experience:

  • High school diploma or equivalent required.
  • Graduate of an accredited school of nursing.
  • Valid state licensure as a LPN.
  • Current AHA Healthcare Provider training (BCLS)

Job Types: Full-time, Part-time, PRN, Per diem

Pay: $25.00 - $35.00 per hour
