The School of Medicine’s Department of Medicine, Division of Infectious Diseases has an opening for a full-time University Staff (unclassified) Healthcare Entry Professional (Linkage and Retention Coordinator) position.

Jobs in the Healthcare career family are responsible for performing a wide range of professional duties to support the university's teaching, research and/or service missions through the provision of health and wellness services, dealing with general, indirect patient care operations and management of healthcare services. Functions include hospital administration, healthcare regulatory compliance, patient relations, operational services, admissions, and medical records. Includes professional assignments in medicine, dentistry, nursing, pharmacy, counseling and behavioral health, public health, dietetics and nutrition, other allied health fields, alternative medicine and therapies, and related disciplines.

Professionals at the entry level are responsible for exercising discretion, analytical skill, personal accountability, and responsibility in a wide range of areas including academic, administrative, managerial and student services functions. Work involves creating, integrating, applying and sharing knowledge directly related to a professional field.

At the entry level, duties are limited in scope and are performed with guidance and direction from other professionals and are performed in a training and development capacity.

This Healthcare Entry Professional is based in the Infectious Disease Group Practice (IDGP) at the University of Colorado Hospital. The IDGP is a busy, fast-paced adult clinic with a large patient population experiencing financial, mental health, substance, social and health related issues. The Healthcare Entry Professional will be operating as a Linkage and Retention Coordinator, overseeing the linkage and retention program by providing supportive services to facilitate linkage, access to, and retention in care for patients in the IDGP.

Examples of Duties Performed

  • Provide case management and psychosocial support to patients in addressing identified barriers and obtaining needed services until successful linkage or reengagement in care
  • Coordinate with care team members to identify patients who are at risk for or have fallen out of care and support their re-engagement
  • Conduct an intake/psychosocial assessment to identify needs and barriers to care
  • Support care coordination activities; including assisting patients with referrals to other support services; including social work, medical case management, mental health, substance use, and adherence counseling when appropriate
  • Network with community agencies and organizations in order to establish referral linkages
  • Primary lead on special HIV testing project with the University of Colorado Hospital emergency department to facilitate timely linkage to care services to the IDGP
  • Primary lead on special retention project with University of Colorado Hospital inpatient services
  • Work as part of the multidisciplinary patient support and engagement in care teams; including participating in team meetings
  • Properly document all linkage and retention in care activities

Minimum Qualifications

  • A bachelor’s degree in health, human services, psychology, counseling, social work, or a directly related field from an accredited institution.
    • Substitution: A combination of education and related technical/paraprofessional experience may be substituted for the bachelor’s degree on a year for year basis.

Applicants must meet minimum qualifications at the time of hire.

Preferred Qualifications

  • Professional experience working in medical setting.
  • Professional experience working with the LGBTQ population.
  • Experience with electronic data capture systems (e.g., EMR or EHR and data management systems).
  • 2 or more years of related professional level experience that includes case management.

Salary and Benefits

The salary range (or hiring range) for this position has been established at $51,378 to $65,353.

The salary of the finalist(s) selected for this role will be set based on a variety of factors, including but not limited to, internal equity, experience, education, specialty, and training.

The above salary range (or hiring range) represents the University’s good faith and reasonable estimate of the range of possible compensation at the time of posting.

This position is eligible for overtime compensation.
