MDS Coordinator Job at Vero Health & Rehab of Amesbury
Vero Health and Rehab seeks an experienced 40 hour MDS nurse to join our great nursing team. The right candidate will have proven OBRA and PPS MDS experience. Working knowledge of CAA's, care planning, ADL scoring and ICD-10 coding is required. Knowledge of the upcoming PDPM medicare reimbursement system is a plus.
Working knowledge of electronic health records (EHR) MDS's and assessments is required.
Duties include but are not limited to:
- Completing assessments, MDS and care plans for assigned residents.
- Works with the MDS director, monitors completion of MDS's by other disciplines ensuring completion within regulatory time frames.
- Communicates with nursing regarding changes in patient condition and ADL status.
- Monitors and audits clinical record documentation to ensure accuracy; ensures that documentation will support the claim.
- Is an active participant in the daily clinical meeting
- Participates in care plan meetings, discharge planning meetings
- Reviews resident accidents/incidents
- Reviews the 24 hour report daily, recommends changes or revisions to care plans as appropriate
