Wickliffe Country Place, a 170-bed skilled nursing facility conveniently located near I-90 and Euclid Avenue, offers a warm and comfortable environment with staff committed to treating everyone like family- with compassion, respect, and kindness. The staff earned the Bronze National Quality Award from the American Health Care Association in 2020. The facility offers multiple specialty programs including on-site dialysis in collaboration with the Centers for Dialysis Care, a high-quality rehabilitation program and a physician-led cardiology program.:

Summary:
The MDS RN or LPN is responsible for the coordination of quality care to residents, from pre-admission through discharge. The MDS RN or LPN will coordinate the written care plan and assessment and coordinate professional nursing care to residents.:

Education and Experience:

Licensed RN or LPN in the State of Ohio:

Prior MDS 3.0 and LTC experience:

MDS Coordinator will interact with many people so it is imperative to have constant professionalism and a pleasant disposition:
