Medical Driver - Techny

Location: Northbrook

Hours: Full Time

Monday-Friday 8am-4pm ; 40 hours/week

Pay: $17.75/hr; PAID Training

+ Sign-on Bonus: Full-Time staff receive total of $2,250 and Part-Time staff receive total of $875 in bonuses the first year of employment with us!

Responsibilities:

• Providing transportation services to the clients of Community Living Services and CILA

• Drive clients to and from their appointments (medical or otherwise) on assigned day

• Escort and attend appointments with the clients as needed.

• Assure safety of clients while being transported.

• Transport clients or supplies needed by clients to their day program.

• Transport ill clients home from work or to another specified location if treatment is

  warranted.

• Provide supervision and training for community skills/self care skills to the client

  according to their program plan.

• Consult with QIDP in regard to client needs, long term goals, and individual objectives.

• Consult with nurse in regard to client’s medical condition(s).

• Ensure proper and accurate information and documentation is received from medical appointment and forwarded to nurse in a timely fashion.

• Refer clients to QIDP for medical appointments, counseling, or other types of specialized assistance.

• Participate in scheduled staff meetings.

• Complete mandatory training annually.

• Exhibit initiative and flexibility in handling unexpected situations, problems, temporary tasks, and emergency situations.

• Provide a respectful, friendly, and supportive atmosphere conducive to personal esteem and growth.

• It is the responsibility of the Driver to maintain confidentiality with all clients and their families and maintain a professional image/role as a representative of the agency.

• Adhere to all policies and procedures established for Clearbrook, the Community Living Services and those of all governing bodies.

• Perform any other duties as assigned.

Requirements:

• High school diploma or equivalent
• Valid driver’s license required
• Must be 21 years of age
• At least 1 year of experience
