Duties and Responsibilities

Pulls charts for scheduled appointments in advance. Inserts out-guides.

Delivers, transports, sorts, and files charts.

Files all reports such as labs, dictation, x-rays and correspondence into Medical Record.

Continually checks for misfiled charts and refiles according to filing system. Maintains orderly files and repairs/replaces damaged file folders.

Looks for lost and misfiled charts, delivers charts to various departments.

Organizes papers, lab results, reports, dictation, etc. in patient file.

Uses computer system to identify chart number in order to properly file incoming documents.

Purges obsolete records and files in storage following established procedures for retention and destruction.

Makes up new patient charts.

Works with nursing and other staff to route patient charts to proper location notifying involved parties when charts cannot be found.

Copies and releases medical records following established policy & procedure regarding release of medical information and confidentiality.

Answers telephone, takes messages, and gives routine information in accordance with established procedures.

Attends required meetings and participates in committees as requested.

Maintains work area in a neat and orderly manner.

Maintains patient confidentiality.

Performs related work as required.

Patient Redesign (applies to East Albany Medical Center)

Ÿ Meticulous grooming of charts for a 100% completeness of medical record.

Ÿ Ensure extra progress notes in chart.

Ÿ Ensures exam rooms are stocked with extra forms for chart.

Ÿ Provides information for team huddles, ensuing that information on incoming correspondence and missing items are communicated to the team.

Knowledge, Skills, & Abilities

Ability to file and maintain patient records, files, reports, and other correspondence. Knowledge of medical records filing systems. Skill in English grammar and spelling. Ability to maintain confidentiality of sensitive information. Attention to detail and accuracy.

Education & Experience

High school diploma or GED.

Completion of course in medical records technology or medical terminology highly desirable.

Previous experience in medical records preferred.

Word processing and computer experience.

Typical Physical Demands

Requires prolonged standing or sitting. Requires frequent bending, stooping or stretching. May require lifting up to 50 pounds. Requires eye-hand coordination and manual dexterity. Requires the ability to distinguish letters or symbols. Requires the use of office equipment, such as computer terminals, telephones and copiers.

Typical Working Conditions

Work is performed in an office environment. Some contact with patients. Deals with situations involving repetitive operations carried out according to set procedures. Occasional evening or weekend work.

Job Type: Full-time

Pay: $10.00 - $20.00 per hour
