WellLife Network is seeking a Psychiatric Nurse Practitioner to join our Huntington Assertive Community Treatment (ACT) program.

ACT is team-based treatment model that provides multidisciplinary, flexible treatment and support to people with severe mental illness.

WHO YOU ARE: The Psychiatric Nurse Practitioner provides psychiatric and medical assessment and treatment; and development, maintenance, and supervision of medication and psychiatric and medical treatment.

SOME ESSENTIAL ACCOUNTABILITIES:

• The nurse practitioner must provide community-based services; the initial assessment visit must be in the community and monthly community visits thereafter.
• Collaborate with the treatment team in comprehensive assessment of psychiatric history, mental status, and diagnosis; physical and dental health; use of drugs or alcohol; education and employment; social development and functioning; activities of daily living and family structure and relationships.
• Collaborate with the nurses in assessment of client physical health, making appropriate referrals to community physicians for further assessment and treatment, and coordinating medical treatments with psychiatric treatment.
• Prescribe psychotropic medications and conduct regular assessments of therapeutic responses and side effects and educate clients regarding psychotropic medications.
• Along with other team members, provide support, education, and counseling to family members of clients to help them become knowledgeable about mental illness, collaborate in the treatment process and assist in their family member’s progress.
• Provide on and off-site crisis assessment and management of patients during regular work hours, both on-site and over the phone, during other hours, and as on-call backup.
• Regularly attend daily team meetings at least 3-4x per week.
• Direct and collaborate with the nurses to develop, revise, maintain and supervise team psychopharmacologic and medical treatment and medication policies and procedures.
• Maintain confidentiality at all times.

Qualifications:

• Currently hold a nurse practitioner certification through New York State education department and experience providing treatment with persons with severe and persistent mental illnesses.
• Clinical supervisory experience is desirable.
• A clear understanding of the characteristics and problems of adults with severe mental illnesses and be knowledgeable about the ACT model.
• Computer proficiency, including the ability to use email, navigate and enter all documentation into electronic medical record.
• Strong interpersonal and communication skills and knowledge of group dynamics required

• Valid New York State driver’s license and vehicle required – candidates are subject to regulatory background checks.
