About Your Work
This position plays a key role at driving clinical initiatives for Population Health, Quality and Transitions of Care initiatives and participates in program appointments, including but not limited to Annual Health Assessments, Chronic Condition Management, or Primary Care appointments.
You will focus on:
- Provides high quality care to patients in collaboration with our clinical team, administration, and staff across the organization
- Leverages the use of the Electronic Health Record (“EHR”) to manage and improve patient care, workflows and clinical compliance.
- Perform certain functions and procedures, which overlap those of physician; details listed out on the Family Nurse Practitioner Standardized Procedures in accordance with The Nurse Practice Act In The California Business and Professions Code, Chapter6 Article2, Section2725, and The California Administrative Code, Title16, Article7, Section1470-4 and Title22, Division3, Section 51240(Medi- Cal)
- Ensures optimal patient outcomes through excellent assessment and coordination of care, patient throughput, optimal length of stay, while ensuring improved patient satisfaction
- Provides direct and indirect patient care as part of the treatment regime, identifying actual or potential risks based on patient screening data.
- May serve as the primary provider of care for patients as appropriate and based on scope of practice.
- Performs patient assessments, H&P’s, and thorough history review
- Orders and interprets laboratory and diagnostic tests
- Develops medical diagnoses, treatment plans, prescribes medical regimes, and educates patient and family with evidence-based practice and technological advances in their scope of practice
- May perform approved procedures and order medications as privileged and in scope of practice
- Collaborates with providers, ancillary personnel, community resources and other interdisciplinary team members to proactively resolve patient needs.
- Participates in or directs data collection, quality improvement, discharge planning, and coordination of care within area of practice.
- Utilizes screening, assessment and treatment data to provide patient/family education
- Travel may be required up to 50% as needed
Requirements for the position:
- Education: Bachelor’s degree or equivalent from 4 year university, preferably in biology, chemistry, physiology, psychology or related field of stud
- Degree and certification from accredited RN program
- Minimum of 5 years required working in a professional experience working as a Nurse with 3+ years as a Nurse Practitioner
- 7+ years of Nursing experience in Primary Care Experience with Geriatrics
- 5 years of experience working with an EMR, preferably EPIC
Preferred:
- 10+ years professional experience working as a Nurse
- 5+ years as an NP
- 8+ experience working with an EMR, preferably EPIC
- 1-2 years working with Geriatric population

Final determination of a successful candidate’s starting pay will vary based on several factors, including, but not limited to education and experience within the job or the industry. The pay scale listed for this position is generally for candidates that meet the specified qualifications and requirements listed on this job description. Additional pay may be determined for those candidates that exceed these specified qualifications and requirements. We provide a competitive compensation package that recognizes your experience, credentials, and education alongside a robust benefits program to meet your needs.
