Synaptic Rehabilitation is a reputable provider of in-home physical, occupational, and speech therapy services to patients throughout New Jersey. We are currently seeking a talented and motivated Occupational Therapist to join our growing team, providing exceptional care to patients in Somerset County and Hunterdon County, New Jersey.

Responsibilities:

• Conduct comprehensive patient assessments to determine their individual needs and develop personalized treatment plans
• Provide one-on-one, in-home occupational therapy services to patients, with a focus on improving their activities of daily living, fine motor skills, and functional capabilities
• Collaborate with the interdisciplinary team, including physical and speech therapists, to ensure a coordinated and comprehensive approach to patient care
• Monitor and document patient progress, adjusting treatment plans as needed to achieve optimal outcomes
• Educate patients and their families on therapeutic techniques and strategies to promote continued progress and independence at home
• Maintain open communication with referring physicians and other healthcare providers to provide updates on patient progress and facilitate seamless transitions in care
• Adhere to all professional, ethical, and regulatory standards governing occupational therapy practice in New Jersey

Requirements:

• A degree from an accredited Occupational Therapy program
• Valid New Jersey Occupational Therapist license
• Reliable transportation to provide in-home services throughout Union County
• Excellent interpersonal and communication skills
• Ability to work independently and manage a flexible schedule
• Commitment to patient-centered care and evidence-based practice
• Prior experience in home healthcare is preferred but not required
