OUTREACH CASE MANAGER
Definition
Professional and direct social service work to the community’s seniors and their families in the areas of aging, housing, care giving, nursing home placement and inter-familial conflict resolution and all other related work, as required. May provide professional assistance to other populations in response to town-wide needs identified by municipal departments.
Supervision
Works under the general direction of the Director of Elder Services. Duties requires independent judgement in the performance of clinical work.
Job Environment
Work is performed in the field as well as in the office; may require some evening, weekend or holiday work. The employee may be exposed to stressful situations and may be subject to unpredictable fluctuations in work volume.
Responsible for operating telephone, computer and mobile devices. Must be able to operate an automobile.
Makes constant contact with the town’s elderly population, health care providers, public, private and non-profit organizations serving the elderly, state agencies and other municipal departments. May work with other populations identified by municipal counterparts engaged in assessing resident needs.
Has access to confidential or sensitive information through personal contacts and counseling.
Errors in judgement could result in the delay or loss of services, adverse effects upon the community or individual serviced, possible personal injury and direct financial and legal repercussions.
Essential Functions

Makes initial and follow-up visits to seniors to identify needs, initiate crisis intervention and case management support services for seniors where health, safety, social or emotional issues require assistance. May provide professional assistance to other populations in response to town-wide needs identified by municipal departments.
Assists seniors in defining their needs, refers clients to appropriate services and facilitates access to services.
Meets with family and/or other caregivers to address physical limitations, personal resources, housing issues and support needed to improve seniors’ quality of life.
Provides accurate and current information regarding the resources available including housing, health care, home health care, homemakers, transportation, fuel assistance, nutrition, and legal and financial resources available.
Provides guidance and assistance to seniors and/or family members in the preparation of forms and documents necessary to obtain services. Maintains adequate and secure records and case files of client interaction. Tracks outreach activities and generates reports to Director when required.
Develops and maintains information and referral networks with various community, health and social service agencies and Town departments. Work with public safety or health officials on visits to homes to resolve immediate emergency situations. Collaborate with agencies/organizations to identify appropriate resources for seniors and their families and caregivers.
This position is a mandated reporter. Provides assessment and follow-up support to include protective services referral.
Provides support to Whipple Senior Center staff in the delivery of successful events and programs. Assist in the development of programs to address emerging needs of the aging population.
Attend and participate in affiliated professional associations/groups to stay abreast of new trends and innovations in the field of senior services.
Performs similar or related work as required.
Recommended Minimum Qualifications
Education and Experience
Bachelor's Degree in Social Work, Counseling, Psychology, Human Services, Gerontology, or a related field. Master’s Degree preferable. Massachusetts Licensed Social Worker (LSW). A minimum of three years of progressive responsible professional experience.
Special Requirements
Position requires a valid Massachusetts Driver’s License and access to a dependable automobile.
Knowledge, Skill and Ability
Knowledge. Thorough knowledge of the federal and state services and local resources available to the elderly. Knowledge of federal and state regulations pertaining to programs for the elderly and reporting laws regarding elder abuse and confidentiality laws in regards to client records. Knowledge of grant writing techniques. Knowledge of elder network, program and services.
Skill. Excellent public relations skills combined with oral and written communication skills. Good public speaking skills. Computer skills to utilize internet software, word processing, spreadsheet and database software applications.
Ability. Ability to develop effective and constructive working relationships with town, State, and federal officials, the elderly, service providers, and the general public. Ability to define problems, collect data, establish facts and draw valid conclusions. Ability to maintain sensitive and confidential information.
Ability to read, analyze and interpret general business periodicals, professional journals, technical procedures and procedure manuals. Ability to communicate effectively both orally and in writing. Must possess compassion for the elderly.
Physical Requirements
The employee is required to stand, walk, sit, talk and hear for extended periods. The employee is required to interact and communicate frequently and effectively with the public. Frequent use of hands to touch,handle or feel objects, tools or controls; reach with hands and arms; stoop, crouch or kneel. Able to operate an automobile. Vision ability of close to medium distance vision and ability to adjust focus in order to read and analyze documents, complete forms and paperwork. The employee must be able to hear normal sounds, distinguish sound as voice patterns and communicate through human speech. May occasionally be required to lift up to forty pounds.
