MAJOR FUNCTIONS:

Under general supervision, the Outreach Worker is essential in locating, engaging and enrolling assigned Health Home eligible persons who have been identified by the NYS Department of Health to be in need of comprehensive and coordinated social services. Outreach activities are defined within reason as electronic searches, correspondence, telephone calls, home visits, field visits and/or agency visits. Transportation is provided for all outreach activities or is reimbursed within the parameters of participating Health Hove providers. The Outreach Worker reports to the Intake Coordinator/HCM Director.

DUTIES AND RESPONSIBILITIES:

  • Performs community based outreach activities such as home visits, provider visits, telephone calls and correspondence.
  • Confirms patient information and coverage via EPACES.
  • Follows engagement activities from an assigned calendar provided monthly.
  • Documents all patient outreach activities in the Health Home EMR (electronic medical record) within two days of encounter.
  • Completes a Time Log reflecting the outreach activity and submits to Intake Coordinator daily.
  • If an Intake is agreed upon with potential enrollee, completes the Demographics Form, A FACT GP, and correct Health Home releases form. Upon return to the office, completes a Time Log and a Progress Note in Health Home EMR that properly reflects enrollment of that member.
  • Prepares and maintains Health Home intake packets for each individual Health Home.
  • Responds to calls from Health Home regarding patients at noted hospital Emergency Rooms.
  • Completes Intakes as assigned and submits all related documentation within two days of enrollment.
  • Completes Bottom-Up Screening Forms as needed.
  • Performs other duties as requested.

QUALIFICATIONS:

  • High school diploma or GED.
  • At least one year of comparable experience

  • Ability to read and write, understand and carry out directions as well as the ability to be organized, detailed and compassionate.
  • Good communications skills
  • Knowledge of community resources, sensitivity towards persons with Chronic illnesses and mental health related conditions.
  • Bi-lingual in Spanish preferred.
  • Must be flexible (24 hour call)
  • Good interpersonal skills
  • Good organizational skills
  • Good computer skills including proficiency with Microsoft Office

Pay: $16.18 per hour
