About the position: The Medical Social Worker is responsible for the implementation of standards of care for medical social work services to Palliative Care and Transitions patients. Services may also be provided to Hospice or Bereavement patients under special circumstances.
Responsibilities:
Patient Care [40%]
· Assesses the psychosocial status of patients and families/caregivers related to the patient’s terminal illness and environment.
· Implements interventions based on assessment findings.
· Counsels patient, family, caregivers as needed re: anxiety and stress related to end of live process. Assists in identifying effective coping mechanisms.
· Provides information and referrals services for patients and families/caregivers regarding practical needs [including but not limited to: funeral arrangements, placement, MediCal applications, traveling contracts]
· Provides EOL education and normalization of EOL experiences and processes
· Provides emotional support and encourages life review for patients and families/caregivers Services as liaison between patients and families/caregivers and community agencies
· Assists physician and other team member in understanding significant social and emotional factors related to health problems and death/dying issues
· Other duties as delegated by the Director of Palliative Care.
Compliance & Risk Management [30%]
· Documents all patient related activities in a timely manner [48 hrs/2 days] in EMR. Participates in POC and regularly updates POC
· Attends regularly scheduled IDG meetings.
· Provides written notes to IDG facilitators when unable to attend an IDG
· Actively participate in quality assessment performance improvement teams and activities as requested.
Collaboration & Communication [20%]
· Works well within a team
· Proficient in effective written, phone and email communication with team members, patients, families, caregivers and other entities serving the patient.
· Communicates assessment findings clearly to team members and IDG
· Maintains collaborative relationships with organization personnel to support patient care
Professional Development [10%]
· Taking on additional responsibilities such as providing 1:1 support for staff and volunteers as needed.
· Taking advantage of continuing education offered by agency. Taking part in agency educational presentations
· Make use of PTO on a regular basis
Qualifications:
· Master’s Degree in Social Work (LCSW a plus but not required)
· Current CPR certification
· One year experience in hospice, hospital, or homecare setting
· Valid California Driver’s License and an automobile that is insured in accordance with state requirements
· Computer proficiency in Microsoft Office and willingness to learn new software as needed
· Team player with excellent organizational, communication, and problem-solving skills
· Bilingual or multilingual preferred

Pay: $37.08 - $52.53 per hour
