Patient Safety and Risk Mitigation Director Job at Humboldt Park Health
Essential Duties and Responsibilities:
- Develop and implement best practices in patient safety and risk mitigation initiatives practices driving a proactive approach and prevention of patient harm.
- Supervises the investigation of errors and then plans for resolution of all safety-related incidents, including those involving employees, patients, and situations posing a threat to the health care setting and equipment.
- Maintains records of patient-related errors and evaluates this information to identify the cause of the error.
- Collects, reviews, and analyzes information from the facility's safety programs to identify corrective action and programs as necessary.
- Recommends changes, policies, or programs that could prevent future errors.
- Designs and presents training programs meant to increase awareness of patient safety initiatives.
- Ensures compliance with state and federal regulatory requirements.
- Typically reports to a head of a unit/department. Manages subordinate staff in the day-to-day performance of their jobs.
- Ensures that project/department milestones/goals are met and adhering to approved budgets.
- For the implementation for the safety through the policies, procedures, and guidance and serve as the facilitator for the program sites providing a strategic, investigative approach in monitoring daily tasks and activities.
- Reviews and analyzes facility occurrence reports identifying clinical risk issues
- Facilitates the investigation and follow-up on events with the appropriate Directors
- Analyzes occurrence data for frequency and severity of events
- Identifies trends necessitating intervention and works with Directors in planning and implementing actions to address identified issues
- Initiates Probable Claims Report (PCR) as appropriate Coaching, counseling, conflict management, consulting, and influencing skills
- Requires extensive experience in patient safety, and risk management, performing these activities in a healthcare setting.
