Job Summary and Qualifications

The Performance Improvement Coordinator ensures quality of patient care by assisting the VP Quality Resources in planning, implementing, coordinating and monitoring all hospital and medical staff performance improvement and peer review activities.

What you will do in this role:

  • Assists Department and Committee Chairmen in the measurement, assessment and improvement of processes.
  • Collects relevant data and identifies records for review by members of the medical and hospital staffs. Uses tools such as CHOIS, Crimson and COMET for data abstraction.
  • Assists in coordination of monthly meetings for regulatory readiness and/or peer review and evaluation of the appropriateness of patient care provided by hospital staff and members of individual medical staff departments.
  • Assists department Chairmen, physicians, and hospital committee members with the investigation of Peer Review, Focused and Ongoing Professional Performance Evaluation reviews, and survey readiness.
  • Develops and maintains format and procedures for documentation and reporting of PI and peer review activities; maintains reports of findings, conclusions and actions resulting from monitoring and evaluation of patient care by hospital staff and members of the medical staff.
  • Provides consultation upon request or as assigned by the VP Quality Resources to any department, coordinator, committee, quality action team or task force regarding methods of quality review, statistics and data analysis; maintains resource materials necessary to assist staff understand and comply with the Performance Improvement process.
  • Assists the VPQ in monitoring the hospital’s progress in meeting the objectives of the comprehensive Performance Improvement program.
  • Promotes compliance with hospital policies, the hospital-wide Performance Improvement Plan and regulatory agency standards to ensure necessary accreditation.
  • Reviews the CHOIS data prior to data finalization; checks to ensure cases with morbidity/mortality have been peer reviewed. Shares outcome measurement results with appropriate individuals/committees and ensures accuracy of data submitted to Joint Commission for ORYX/Core Measures.
  • Conducts case root cause analysis for all core measure fall-outs, and educational team builds for nursing and medical staff.
  • Collaborates with others on the Quality team, for successful project management, such as, but not limited to: LeapFrog, regulatory surveys, readiness surveys, quality reports…
  • Accept and maintain the National Association for Healthcare Quality’s (NAHQ) six areas of quality competencies: health data analytics, population health and care transitions, performance improvement and process improvement, quality review and accountability, regulatory and accreditation, and patient safety.
  • Complete other duties as determined necessary and delegated by the VPQ.

What qualifications you will need:

  • Graduation from an accredited school of Registered Nursing is required
  • Must be licensed as a Registered Nurse in the state which you practice and/or hold an active Compact/Multi-State RN license
  • 1-3 years quality management and performance improvement required
  • 3-5 years clinical experience required
  • Certified Professional in Healthcare Quality (CPHQ) preferred
  • This role requires you to be fully vaccinated for COVID-19 based on local, state and /or federal law or regulations (unless a medical or religious exemption is approved).
