Personal Care Aide/Private Duty Job at Concordia Lutheran Ministries
Concordia of the South Hills, located on 19 acres on Bower Hill Road in Mt. Lebanon, offers a beautiful environment situated just a few minutes from shopping, cultural events, and downtown Pittsburgh. This continuing care retirement community consists of retirement living apartments, personal care suites, memory support personal care suites and skilled nursing - which means those in our care have the opportunity to age in place. QUALIFICATIONS
A. Provide personal care including but not limited to:
Bathing
Dressing
Grooming
Hair Care
Nail Care
Assistance with ambulation
Changing position in bed
Assistance with transfers
Oral hygiene
Changing bed linens of an incontinent beneficiary
Shaving
Deodorant application
Skin care with lotions and/or powder
Foot care
Ear care
Feeding
B. Housekeeping including but not limited to:
Dusting
Vacuuming
Light meal preparation
Taking out the trash.
Laundry
C. Assistance with medications that are ordinarily self-administered and do not require the skills of a licensed nurse to be provided safely and effectively including:
Handing the bottle to the patient
Opening the bottle
Handing the patient water or other liquid to take the medication
Reminding the patient to take a prescribed medication
D. Assistance with activities that are directly supportive of skilled therapy services but do not require the skills of a therapist to be safely and effectively performed. Examples of such activities include:
Routine maintenance exercises
Repetitive speech routines to support speech-language pathology services
E. Routine care of prosthetic and orthotic devices
F. Performs all duties as assigned.
Documentation
Accurately records services in telephony system as instructed
Communication
A. Reports any changes in the patient’s mental or physical condition or the home situation to the service coordinator
B. Maintains confidentiality of patient/client information according to HIPAA
Professional Standards
Complies with infection control and safety policy procedures
Adheres to all policies and practices of the agency to maintain established Standards of Care
Carries out only those activities for which she/he has been trained and supervised
Maintains a professional standard of conduct, projecting a positive image of the agency at all times
Works with patients in a caring, therapeutic and a Professional manner at all times

QUALIFICATIONS
- Successfully completes Agency Competency Evaluation
- One year of supervised employment as a personal care aide or nursing assistant is preferred
- Scores at or above the 75% score on the nursing assistant evaluation/pre-employment test
- Ability to understand and carry out directions and instructions and keep appropriate records
- Hold current CPR and First Aid card (HSFA)
