The Personal Care Worker is responsible for the general operation of a group home/supported apartment settings for adults with developmental disabilities and/or chronic mental illness.

Education and Experience Requirements

Required: Ability to effectively communicate

Valid Wisconsin driver's license and appropriate auto insurance coverage as required by North Central

Health Care.

Preferred: One to three years relevant experience

Any combination of education and experience that provides equivalent knowledge, skills and abilities may be considered.


Essential Duties and Responsibilities:

  • Creates a supportive, family-like environment in order for each resident to achieve maximum independent functioning based on individual needs.
  • Provides home services (meal planning and preparation, housekeeping, laundry, help members with accounting, etc.)
  • Assists as needed with ongoing assessment of residents’ needs.
  • Communicates observations relating to functionality of residents.
  • Provides client supervision/support in the use of social/recreational and other community services (shopping, public transportation, etc.).
  • Monitors residents’ health needs including administration of medications, providing first aid and obtaining medical services when necessary.
  • Maintains regular and predictable on-site attendance.
  • Other Responsibilities:
  • Perform other work related tasks as required as assigned

  • Complete organizational, leadership, management, clinical, department, program, common position and job specific education as required by North Central Health Care and/or various regulatory agencies based on job title.
  • Qualifications and provision of care are in accordance with national acceptable standards of practices pertinent to role based on job title.
  • Knowledge of regulatory and accreditation standards along with federal, state and local codes pertinent to role based on job title.

Physical and Working Environment

  • Heavy Work: Frequent lifting. Must be able to lift a minimum of 50 pounds unassisted.
