Physical Therapist Assistant
Status: Pool Status Hourly Non-Exempt

We are a 24 hours skilled nursing facility dedicated to providing quality care to our residents with 62 bedrooms, which includes 44 private rooms. Our services include 24 hours nursing care, Medicare certified beds, physical, occupational, and speech therapy and so much more.
Summary Description: The primary function of the Physical Therapist Assistant (PTA) is to deliver quality care to our residents by providing comprehensive Physical therapy services. This includes the evaluation, development, implementation and documentation of resident treatment.

Qualifications: In order to be considered for the position you must meet the minimum qualifications listed below.
Education:
- High School diploma or Equivalent
- Ability to read, speak, and write English
- Currently licensed to practice Physical Therapy in the state of Wisconsin.
- Associate Degree in Physical Therapy from an accredited institution.
- Previous long-term care experience, preferably:
- Demonstrates interest and positive attitude and patients about working with long-term care residents.
- This work is performed indoors, in the rehabilitation department and throughout the facility, but occasionally outdoors or in the community.
Specific Requirements:
- Ability to maintain professional interaction and communicate effectively and appropriately with residents, families, community members, volunteers and all other facility staff.
- Demonstration of professionalism at all times
Physical Requirements:
- Meet facility policies and procedures regarding necessary physical health.
- Bending/stooping, standing/walking and lifting must be met to be employed due to the essential responsibilities includes 75% of the physical activities.
- Basic computer skills with knowledge of basic Word, Excel, and MS office.
- Effectively delivers appropriate treatment procedures to a variety of diagnoses in accordance with established treatment programs. Treatment procedures will be appropriate to the disability treated and goals established.
- Accurately documents objective medical findings, goals and plan of care according to established department policy. Completes required documentation for all insurance carriers in a timely manner. Takes responsibility that all documentation is properly co-signed by a Physical Therapist.
- Works closely with other therapists, assistants, technicians and restorative aides in the department.
- Share responsibilities of maintenance of clinical equipment, linen and departmental supplies
- Completes appropriate documentation of resident’s participation.
- All other duties assigned as needed as directed from other rehab professionals and/or Director of Rehab.
