JOB SUMMARY

Sky View Rehabilitation and Health Care Center, a 192-bed Skilled Nursing Facility located in Croton-on-Hudson, has openings for Full-Time Physical Therapists. New grads welcome! Duties include, but are not limited to:

• Must possess the ability to plan, develop, organize, evaluate and implement the programs, goals, objectives, policies and procedures, etc., of the physical therapy program to help residents obtain/maintain their highest level of physical, mental and social well-being
• Interpret and/or instruct in physical therapy policies and procedures to personnel, residents, family members, etc as necessary.
• Participate in discharge planning, development, and implementation of resident care plans, resident assessments (MDS) etc as necessary.
• Supervise Physical Therapy Assistants as outlined by the NYS Physical Therapy Practice Act.
• Maintain treatment records, resident files, and progress notes as required.
• Maintain an adequate liaison with families, residents and interdisciplinary team members.

• Long term care experience a plus!
• Must possess a current, unencumbered license and registration to practice as a Physical Therapist in the State of New York.
• Must possess current malpractice insurance.
