POSITION SUMMARY

The primary function of the Population Health Program Manager is to support the coordination, performance, and evaluation of assigned services and value-based programs. This role will work in collaboration with multidisciplinary teams internal and external to Riverwood. This individual will serve to enhance population health by supporting the integration of patient care services with community resources and ensuring key deliverables are achieved for our value-based programs. This role works collaboratively with others to restore health and promote wellness by coordinating care of patients of all ages, gender, economic and social status, and backgrounds.


POSITION QUALIFICATIONS

Education and Experience

  • Bachelor's degree in Business Administration, Healthcare Administration, Nursing or related field required. Two (2) or more years of experience or an equivalent combination of education and experience sufficient to successfully perform the essential responsibilities and duties of the job.
  • Minimum of (1) year experience in project/program management preferred.
  • 2-3 years of experience with population health, value-based contracts, and/or ACO experience strongly preferred.
  • Clinical experience or operational experience in a clinic setting preferred.

Knowledge

  • Knowledge of healthcare regulatory bodies and relevant quality improvement standards and metrics.
  • Knowledge of nursing processes; health care systems, structure, and functions; and community resources.
  • Knowledge of chart/medical record documentation requirements and federal/state laws related to release of health care information.

Skills

  • Skill in time management, problem solving, multitasking, prioritizing, and care coordination.
  • Advanced skills in Microsoft Office Suite, electronic health record (Epic), and data reporting platforms.
  • Skill in preparing/maintaining records, writing reports, and responding to correspondence.
  • Skill in both internal and external relationship management with all levels of expertise from administrative to clinical.

License/Certificates

  • If applicable, current unrestricted Minnesota RN license required. BLS required.

ESSENTIAL DUTIES & RESPONSIBILITIES

  • Program Management:
  • Complete day-day tasks and provide program oversight of value-based initiatives, including but not limited to:
    • Accountable Care Organization (ACO) partnerships
      • Medicare Shared Savings Program
      • Integrated Health Partnership
    • Medicare Advantage Quality Incentive Program
  • Define methods for measuring program success and demonstrating program effectiveness in managing total cost of care.
  • Maintain knowledge and understanding of value-based initiatives and day-day clinic workflows.
  • Plans and supports initiatives in conjunction with interdisciplinary teams.
    • Defines project scope, completes user analysis, and establishes goals and deliverables
    • Tracks and ensures that goals, objectives, budgets, and timelines are being met
    • Conducts post project evaluation
  • Assist with special projects as assigned. Typical projects are cross-functional, involving shared responsibility and problem solving with disciplines and staff from different areas.


  • Care Integration & Community Engagement:
  • Responsible for the development and maintenance of standardized outreach efforts that will:
    • Enhance patient engagement and care coordination
    • Educate patients on preventive care and age-related screenings
    • Help identify social determinants of health and reduce barriers to care
  • Complete post-hospital discharge phone calls to support transitional care needs across the patient care continuum. May collaborate with patients' caregivers to improve patient outcomes during and after discharge. Duties to be assigned by licensure.
  • Support the coordination of health-related programs/events for the communities served by the organization.
  • In collaboration with community stakeholders, help maintain Riverwood’s online community resource platform (Riverwood Connects).

  • Data Analytics/Quality Improvement/Reporting:
  • Maintain knowledge of relevant quality measures and specifications to ensure clinical workflows are aligned to enhance optimal outcomes.
  • In collaboration with data analytics and IT team, ensure the electronic health record (Epic) reports, dashboards, and data interface requirements are optimized and maintained.
  • Assist with quality improvement and population health outreach activities as assigned.
  • Collaborates with Primary Care Services manager on clinical process performance improvement initiatives, including data collection, analysis, display, and interpretation.
  • Integrate the use of performance reports and dashboards to identify process improvement opportunities.
  • Manage chronic condition and payor specific registries within electronic health record (Epic) and external platforms to ensure optimal performance with quality outcomes.
